Patient's Query
Hello doctor,
I received a blowjob on my penis from a female of unknown status. There was no other exposure other than this. There was no visible blood and no ejaculation.
What is my HIV risk from only receiving oral sex and not giving?
I also did a 4th-generation HIV combo test on the 30th day after this exposure, and the result was non-reactive. Am I safe, or do I still have to test again for this exposure?
Have read on some reputed websites like HIV i-Base, CATIE, and POZ Forum that receiving a blowjob is zero risk for the receiver and a very low risk for the giver. Is it true, or are these websites providing misleading information?
Kindly help.
Hello,
Welcome to icliniq.com.
I understand your concern.
As per your shared details, I would tell you that the forums are mostly correct that the maximum risk occurs to a recipient and not the giver, but there are some exceptions here.
Although, as per some studies and literature, the risk is very nominal in your case (one to three in 100), let us consider the following points:
Human immunodeficiency virus is very fragile to survive in the open.
The lady must have any type of lesions in her mouth such that her saliva has direct contact with her blood (blood-mixed saliva); otherwise, human immunodeficiency virus has not been found in salivary fluids until now.
It will be good if you trace her and get her tested too (just for your peace of mind).
Intact skin (not mucosa) is the largest barrier for human immunodeficiency virus that prevents its entry into the body.
Do get tested on the 90th day if you are still doubtful about the lady's status.
You should not have any open wound/lesion on your penile skin. Check it once.
Consider the above points and stay calm; do not worry. Please avoid unprotected sexual contact and do follow a healthy lifestyle.
Avoid any tattoos, sexual contact, and blood donation until you are fully free of your doubt of being seronegative. Please contact your nearest Infectious disease specialist for any examinations.
I hope you are satisfied with my answer.

Thank you.
Patient's Query
Hello, Doctor,
Thanks for the reply, Doctor.
You said the maximum risk occurs for the recipient and not the giver, whereas the forums are saying just the opposite. Also, you have told me to test on the 90th day. What is the window period of HIV 4th Gen combo tests?
Kindly help.
Hello,
Welcome to icliniq.com.
I understand your concern.
The window period of the chemiluminescent microparticle immunoassay (CMIA) test (which detects antigen or antibodies) you took is from 2 weeks to 45 days. The 90th day I recommended will be just for your confirmation and to rule out all the other possibilities of antibodies not being detected.
Thank you.
