Can a fourth-generation HIV test accurately assess HIV?

Hello doctor,

I am 37 years old. 48 days ago, I had protected vaginal intercourse with a sex worker for about two minutes. I underwent two fourth-generation tests on days 14 and 25, and both were negative. The laboratory result values were "0.22" using the CLIA (Clinical Laboratory Improvement Amendments)method. I have a lot of anxiety about the possibility of being infected with HIV (Human Immunodeficiency Virus).

Kindly advise.

I understand your concern and would be happy to help. The fear of acquiring HIV is quite disturbing. I have reviewed your reports (attachment removed to protect the patient's identity). It is negative. These test results are based on all or none phenomenon, which means any value above the cut-off is positive, and any value below the cut-off is negative. It is a fourth-generation test done between three to four weeks of exposure, so it can be considered conclusive.

A fourth-generation HIV test is considered accurate within three to four weeks of exposure, provided there has been no further risk of re-exposure. However, if you are still concerned, I suggest you go for ELISA (enzyme-linked immunosorbent assay) test at 12 weeks. I also suggest you get HBsAg (hepatitis B surface antigen test), anti HCV (hepatitis C antibody test), and VDRL (venereal disease research laboratory test) done.
