Hello doctor,

I have an itchy rash and blisters on my hands, feet, and legs that started two days ago. They have cleared up on my hands but are still there on my feet and lower legs. I am not allergic to poison ivy. I want to just figure out what it is so I can avoid it in the future.

I have reviewed the image you shared (attachment removed to protect the patient's identity).

After going through the history and the image, it appears to be urticarial hives that have occurred as a response to exposure to a plant or a leaf. Nettle leaf exposure commonly appears as grouped papules or rashes on the exposed area. Rashes may also appear due to insect bite reactions or other allergens like pollutants, paint, polishes, or cloth fibers. However, the rashes shown in the image appear to be in response to a hypersensitivity reaction to a plant family.

I suggest the below advise:

  • Keep a note of any exposure to a plant, insect bites, or any pollutants in general in the past three days following which the rashes have appeared. It will help prevent further exposure and rashes. Also, keep a note of any food allergens or allergic reactions to any particular medication to prevent reactions to them as well.
  • Mometasone cream can be applied externally on the affected area once daily at night in a minimal quantity. It can be continued for a maximum period of three weeks or till the rash is reduced, whichever comes first. It helps in reducing inflammation, redness, and itching.
  • Tablet Cetirizine 10 mg to be taken at night for one week. It helps in faster reduction of the current rashes and also prevents itching. It is advised to avoid going out at night or driving, as this tablet can cause a sedative effect. It must be taken an hour before going to bed.
  • Calamine lotion may also be used multiple times daily on the affected area to reduce itching and inflammation associated with rashes.

Consult a specialist doctor, talk to him or her, and start the medicines with their consent.
