Hello doctor,

My girlfriend is asymptomatic but has IgG antibodies for HSV 1 and 2. She does have oral lesions sometimes but never genital. I am HSV 1, HSV 2, IgG, and IgM negative. What are my chances of getting it via sexual intercourse? What is the lowest my risk can be and how?

Thank you for the query. Herpes simplex virus 1 or 2 can be transmitted by skin-to-skin contact or kissing or as STD (sexually transmitted infection). The chances are higher when there are active lesions. Avoidance of contact especially around this time may be of some help. Please check whether the partner is positive for both HSV 1 and 2 or 1 of the antibodies. It usually does not matter whether 1 or 2 for treatment. As per the description, the site of the lesion is oral. There are high possibilities of transmission if she is a regular partner. Suppressive antiviral therapy can be considered for your partner to reduce the chances of transmission. You can consult and discuss with your doctor about it. HSV is not a very serious disease to worry about but is known for recurrence.

What is the percent risk if she takes Acyclovir and we use condoms?

Thank you for the follow-up query. It is around the oral cavity. So condoms may not be of help unless oral sex is involved. Moreover, the condom is not very much protective against herpes as there may be areas involved other than condom-covered areas. Acyclovir suppressive therapy does help to prevent recurrence. Transmission may not be fully prevented.
