Hi doctor,
I have felt a little discomfort on my left testicle for about five to six years now. It is not a pain, more of a discomfort. Three years ago, I did an ultrasound on it, and everything was normal. This discomfort comes and goes. Usually, it goes off for about five to six months, and then it comes back and stays for around one month. I masturbate a lot (two to three times a day, almost every day for years now), and I wonder if this could be the cause?

I hope you have done an ultrasound with color Doppler and ruled out varicocele. It could be because of mild inflammation in the testis, which sometimes got aggravated while touching it. Try to wear scrotal support most of the time, especially when you are in an upright position.

Is there any self-examination to see if I could have varicocele or only ultrasound is available to diagnose varicocele? I heard some self-exam where you blow in your hand to see if the testicles expand. Kindly help.

It is difficult to see on self-examination. Only high grades or grade 3 or 4 varicocele could be found on examination. You can get it checked by a local doctor, who will examine you when you do the Valsalva maneuver (forceful attempted exhalation against closed airway). But again, it will be confirmed only on color Doppler examination.
