Hi doctor,

I thought I had thrush due to antibiotics for UTI. But I think it is either a severe yeast infection or ingrown hairs. I have one small lump with a red sore and a smaller one next to it with a fissure at the top near my clitoris. I had an ingrown hair last week and pulled it out, and it resolved, but this is painful when pressure is applied. I have had a bit of cottage cheese-like discharge, which I know is thrush, but I did take over-the-counter drugs, and it has not resolved it. I have managed to tweeze some of the hairs out of the lump or sore. It feels like it has alleviated some pressure. However, I am concerned if it is infected. I have previously had swabs for STIs recently, and they came back fine. I always wear protection, and I have had the same partner. I have been applying Canesten cream and tea tree oil. This comes up overnight, maybe two days. I do shave quite regularly. Currently, I am taking Sertraline. Please help.

From your history and the picture (attachments removed to protect the patient's identity), I think it is an infection of ingrown hairs. Also, you have caused trauma in that region with a tweezer. Two minor lacerations are seen. This is the cause of the pain. Things you need to do,

1) Apply warm compress after thoroughly washing the area.

2) Do not wax or use any hair removal agent. The pubic hair protects from many unwanted infections; also, regular removal may irritate the area.

3)Remove the dead skin (seen as white flakes with gentle exfoliation like a face scrub or pH-balanced washes).

4)Continue Canesten cream (Clotrimazole) and tea tree oil.

5) Discontinue the use of underwear at night.

If your complaint persists, kindly visit a gynecologist or dermatologist.
