Hi doctor,

I have not had a period in years. I have taken Provera a few times to bring on a period, but that is it. I took 50 mg of Clomid, and I did not ovulate on it at all. I am 21 years old right now. I really do not understand what is going on with my body. What medication should I take to try to conceive if I do not have periods or ovulate on my own?

I read your query and understand your concern.

With this limited information, it is difficult to come to a diagnosis. A few facts about the possibilities are:

1. Check your height and weight to know if you are overweight (BMI more than 24 kg/m2) or underweight (BMI less than 14 kg/m2). For this, you will need to correct your weight as per the BMI (body mass index).

2. Check if your secondary sexual characteristics like breast development, growth of axillary and pubic hair, female body contour, and voice have all developed well. If not, then you need an endocrine and karyotype evaluation.

3. Have you taken any scans of the abdomen and pelvis? What is the size of the uterus and ovaries? Are there any renal problems?

4. I suggest you do a hormonal evaluation if all the above tests are normal. Also check for serum FSH (follicle-stimulating hormone), LH (luteinizing hormone) on day 3 of periods, serum AMH (anti-mullerian hormone), thyroid profile, serum prolactin, and serum estradiol.

If you had to take only Provera (Medroxyprogesterone acetate) to get periods, that means your uterus is responding and normal. Most of the time, there is no ovulation due to some other cause. I am sure you will find the reason in one of the mentioned tests.
