Hi doctor,

I had ongoing stomach issues for a while now varies from stomach cramps to diarrhea off and on for around six to eight months. It may clear up for a few days, but it will be back. I have tried various drugs like Imodium, Buscopan. Off and on, I have tried changing my diet to avoid certain foods; there does not seem to be much difference. I have blood work done recently to check for a stomach ulcer, but it came back normal.

Looking at your description of symptoms, it seems that you have irritable bowel syndrome, which is a common condition of the hypersensitive gut that causes pain, altered bowel habits, diarrhea in some, and constipation in others.

But I want some more pertinent information before jumping to establish this diagnosis. Since irritable bowel syndrome is a diagnosis of exclusion, there is no other condition or disease that could explain your symptoms.

Kindly answer my following questions.

  • What do you mean by diarrhea?
  • Are these loose or unformed stools increased in frequency that is more than three times in a day?
  • Are the stools large or small in volume?
  • Does a large volume mean more than 250 to 300 ml (a full glass of water)?
  • Do you feel a repetitive urge to pass stools, but nothing comes really?
  • Have you lost weight in the last six to eight months?
  • Do you ever notice blood in stools?
  • Do you have a family history of colon, stomach, or abdominal cancers?
  • Do you feel any particular food causing this, like milk or wheat or whole wheat or whole grain or nuts or fish?
  • Do you ever been told that you have food allergies or milk intolerance?
  • Do you feel this cramp-like pain resolve after you pass stools?
  • Do you have these cramp or bowel movements troubled you in the middle of the night while asleep?
  • Do you smoke or use alcohol?

I would appreciate it if you could provide me the above information. It will help us to establish that it is IBS (irritable bowel syndrome) or anything else.

Do upload blood tests and any other test you have gone through since your symptoms.

IBS. Celiac disease. Food allergies. Lactose intolerance.

Thanks for your quick reply.

I have loose stool every day, so often as a liquid. I will spend quite a lot of time in the toilet in the morning, for about two or three hours. Later all of a sudden, I need to leave for work and go to the toilet. There is no blood in the stool that I have ever noticed. And no family history. I have booked an intolerance test but cannot get it for three to four weeks, and I am getting down over these issues. Quite often, even once, I have been to the toilet while the stomach is still feeling bloated, gassy and could need to go to the toilet again quite soon after. And there are no issues during the night usually. I do not smoke or drink alcohol.

I can imagine that you are very concerned about this ongoing problem. Well, it highly seems irritable bowel syndrome diarrhea-predominant IBS-D.

The criteria of IBS is,

  1. Abdominal cramp or pain which changes with the passing of wind or stools.
  2. Duration for six months, at least.

No other cause is explaining symptoms (I would suggest work up for it below).

Change in stool form and quantity.

You see, all these things are present in you.

I suggest you take

1. Tablet Mebever (Mebeverine) 200 mg twice daily 20 minutes before breakfast and dinner.

2. You need to avoid gas-producing diets. In medical terms, we call it high FODMPAs (Fermentable Oligo, Di, Mono-saccharides, and Polyols) diet.

3. Your weight seems to be a problem. Since the fat over the abdomen causes compression of the stomach and bowel, it causes excessive bloating and feeling of gas.

I expect if you would follow a low diet and low caloric diet, FODMPAs, with some regular exercise, preferably via joining a gym, would make your problem solved in the long term.

I have seen your blood report; they seem alright except for low folate value, but that does not seem to be a problem.

Please get some stools tests, not urgent, but we need to confirm that this is nothing other than IBS.

  • Stool for calprotectin.
  • Stool detail report and ova parasites.
  • Stool for spot Na (Sodium) and Spot K (Potassium).
  • Blood anti-tTG (anti-transglutaminase) IgA (Immunoglobulin A) and IgG (Immunoglobulin G).
  • Serum IgE (Immunoglobulin E) levels.
  • Serum Na and K.
  • Serum osmolarity.

Hi doctor,

Thanks for the response.

How do I go about getting the Mebeverine? As I am guessing, it is a prescription only. I am sorry I should have mentioned I am a bodybuilder, so carry a lot more muscle mass than average, I know a high protein diet can sometimes be an issue, but I have cut food right back and swapped things around to see if any difference but it has not helped. I will have a look at the food map and see if anything helps. Also, the stool sample is from my GP? As I am not having much luck with them, that is why I have come to this site. I was also advised by another doctor to use an H2-blocker to see if this helps as was having random flare-ups of acid reflux, but I cannot buy this over the counter, and as I say, my GP has not been much help.

Fine, I understand. Unfortunately, the medicines we recommend are not in the form of a prescription, nor that prescription is valid cross country wise. But Mebervine in large parts of the world is over the counter. I hope it is available in your country as well without a prescription. For those tests, we can wait. They are not urgent. You can get when you feel comfortable and feasible.
