Hello doctor,

I am a 33-year-old man, and I had unprotected sex with a sex worker about two weeks ago. Now, when I urinate, I experience a burning sensation, and there is a discharge from my penis. There is this strange itching in that area. I am quite concerned, honestly. Can you explain what might be happening and advise me on what I should do about it? It is all just too much for me to figure out.

If there was unprotected sex, one needs to consider the possibility of a sexually transmitted infection:

  1. TPHA (Treponema pallidum hemagglutination).
  2. HPV (human papillomavirus) DNA PCR (polymerase chain reaction) test.
  3. Herpes simplex virus (HSV) one and two IgG (immunoglobulin G).
  4. Herpes simplex virus (HSV) one and two IgM ( (immunoglobulin M), serum.
  5. Chlamydia trachomatis IgG.
  6. HIV one and two antibodies, screening test.
  7. HbsAg (hepatitis B surface antigen).
  8. .Urine complete.
  9. If not, it could be a simple bacterial infection. You can use T Bact (Mupirocin) ointment for local application and Doxy (Doxycycline) 100 mg twice a day for 5 days. If there is itching, you need to consider a fungal infection. Use Candid B (a combination of Beclometasone and Clotrimazole) ointment for local application, and for itching, take tablet cetzin (cetirizine) once a night. Make sure you wash your genitalia twice a day, morning and night, using normal water (not too cold or hot). Avoid using heavy pH soap.
