I am experiencing intense itching on my scrotum, penis, and along the sides of my thighs. Additionally, there are some hard pimples on my scrotum. This started a week ago. I used clobetasol propionate cream, but there has been no improvement.

After reviewing the photographs of the affected area, my final diagnosis is scabies with scabietic nodules. I am prescribing the following treatment:

  1. Apply Permethrin 5% lotion to the entire body below the neck, leave it on overnight, and wash it off the next morning. Repeat this application after one a week.
  2. Apply a cream containing hydrocortisone and crotamiton twice daily (morning and evening) over the nodules.
  3. Take Levocetirizine 5 mg in the evening.

Please follow up for a review after 15 days.

Hello doctor,

My father, who is 44 years old, has been managing diabetes for the past six years. His condition has been under control with the help of medications, Paciwin and Metformin 500 mg BD. However, today his glucose level was 242 mg/dL, which is higher than usual.

A few days ago, he experienced some discomfort while wearing shoes, and now we have noticed a whitish scar-like formation between the lateral two digits of his right foot. I have attached a photograph for your reference.

Could you kindly advise on what might be the issue and the next steps for treatment?

I saw the pictures (attachment removed to protect the patient's identity). It seems that they are having fungal infections again. It is called pityriasis versicolor.

Here are my suggestions to you:

  1. Tablet Fluconazole 400 mg stat given followed by a week.
  2. Luliconazole cream for local application in the evening and morning over the affected area.

Thanks for responding.

My father is a diabetic patient from last six years but glucose levels on average are under control but today glucose level was bit high that is 242 mg/dL. He is having Cetapin 500 mg BD for diabetes and he is also having Paciwin. He is 44 years old. Few days back he started feeling a bit uneasiness in shoes and now in between lateral two digits of his right feet is a whitish scar type formed please see the attached photo.
