I am having intense itching on my scrotum and penis and along the sides of my thighs, and also, some hard pimples are there on my scrotum. It occurred a week ago. I used Clobetasol propionate G, and there was no difference. Please help.

As per your history, there are two possibilities either you have scabies or fungal infection, and treatments for both are different. Before deciding on the treatment, I suggest you send me a photograph of the affected area.

After seeing the photographs (attachment removed to protect patient identity) of the affected region, my final diagnosis is of scabies with scabietic nodules. I am prescribing the following medications. Use Permethrin 5 % lotion application below the neck over the whole body and then wash off the next morning and repeat the same after one week. Over the nodules, apply Crotorax HC (Hydrocortisone and Crotamiton) cream twice daily applications at night and morning over the affected area. Take a tablet of Levocetirizine 5 mg in the evening. Review after 15 days.

Yes, the lotion is to be applied once a week. Permethrin is a generic name, and the brand name is Klmite cream or Pelol cream.

My father has been a diabetic patient for the last six years but is under control and has been having medications Paciwin and Metformin 500 mg BD for diabetes. The glucose level today was 242 mg/dL, which was elevated than other days. He is 44 years old. A few days ago, he was feeling uneasy wearing shoes, and now in between the lateral two digits of his right foot, some whitish scar type formed. Please see the attached photograph and advise what is right.

I saw the pictures (attachment removed to protect the patient's identity). It seems that they are having fungal infections again. It is called pityriasis versicolor. I suggest tablet Fluconazole 400 mg stat given followed by a week and Luliconazole cream for local application in the evening and morning over the affected area.

The photographs (attachment removed to protect the patient's identity) showed soft corn and interdigital intertrigo. I suggest you use Candid cream (Clotrimazole 1 %, Benzyl alcohol, and Methylparaben) for local application in the evening and morning.
