Can a positive APLA test lead to IVF failure?

Patient's Query

Hello doctor,

I underwent an in vitro fertilization (IVF), and it was a failure. In the last two years, I have never conceived, and just before the IVF test, I was told I am APLA positive with an anti-beta-2-glycoprotein IgM value of 105.2mg/dL. I have been prescribed tablets HCQ 200 mg, Ecosprin 75 mg, and Folic acid.

Can I conceive in the future?

Answered by Dr. Monica Mathur

Hello,

Welcome to icliniq.com.

APLA (antiphospholipid antibody) test basically diagnoses certain antibodies which stop the embryo from growing. This is basically a bi-chemical test, and since you have undergone one IVF (in vitro fertilization), I would like to know whether your both tubes are open, if you have any abnormality in your uterus, and if you are ovulating or not.

Please revert with the queries so I can elaborate on the topic.

Thank you.

Patient's Query

Hello doctor,

My tubes, uterus, and endometrium are perfect and have no issues. I have a low ovarian reserve and recently was APLA positive. I was given FSH injections and have pooled three embryos. One embryo was transferred this month which turned out negative. The main concern is that I have never seen a positive pregnancy in the last three years, and even after trying IVF, there is an implantation failure. I am not sure whether it is because of APLA's positive result or something else. The rheumatologist suggested I repeat the APLA test six weeks later to see if the values are decreasing. Will my next transfer be successful if the APLA values come down?

Hello,

Welcome back to icliniq.com.

The APLA is positive, which is why your IVF is rejected. So once the APLA levels are down, the IVF should be performed, and there are chances of you getting pregnant.

Thank you.

Patient's Query

Hello doctor,

How long does it take to reduce the APLA values? And is the treatment given appropriate?

Thank you.
