Hello doctor,

My skin in the inner thigh section around my penis and inner thighs is very bad, I have been scratching it for almost two months as I face severe itching sensation there, but now the entire area is so black and ugly. The skin is dead, and I also had ringworm around the area three years ago. I do not know what to do. Kindly help.

I have gone through the information provided on your end, and I understand your concern. The rashes, as per the history provided, may be Tinea cruris or fungal infection on the areas which are closed and unexposed. It usually forms due to stagnation of moisture and sweat and reduced immunity. As you have had ringworm in the past, it could also recur in case of low immunity. I suggest the following:

1. Capsule Itraconazole (Sucrose, Maize starch, and purified water) 200 mg once daily after dinner for three weeks.

2. Terbinafine (Terbinafine hydrochloride) cream 1 % in the morning and night on affected areas for eight to twelve weeks or till rashes reduce and pigmentation lessens in appearance.

3. Capsule Becosules (Calcium pantothenate 50 mg, Vitamin B12 15 mcg, Folic acid 1.5 mg, Thiamine mononitrate 10 mg, Riboflavin 10 mg, Pyridoxine hydrochloride 3 mg, Niacinamide 100 mg, Ascorbic acid 150 mg, Biotin 100 mcg) Z once daily after breakfast for one month to increase immunity.

4. Abzorb (Clotrimazole) dusting powder every four hours between cream applications for more than two months.

5. Kz (Ketoconazole) soap for bathing. Use on affected areas only; lather with water and apply on affected areas and leave for three minutes and wash off. Use regular soap on unaffected areas.

6. Avoid stagnation of moisture and sweat to prevent further recurrences of the same rashes.

7. Dry the area well with a towel after bathing and air the area well under the fan daily.

8. Wear loose undergarments. Consult your specialist before taking medications.

Reducing rashes will take time, and consistency with the medication is key. Kindly continue till the rashes reduce. I hope I have cleared all your doubts.
