Hello doctor,

A friend of mine has a known history of hypertension and diabetes. Recently, he developed inflammation and swelling in the right middle finger joint. He was prescribed Zyloric 100 mg and underwent some tests. I will attach the test results for your consideration. He was diagnosed with diabetes 18 years ago and is taking Janumet 50 or 500 mg, which is well-controlled, and he has no food or drug allergies. One month ago, he had the following tests done: HbA1c (6.2) and uric acid level (471). Although he was diagnosed with high uric acid levels last year, he was not taking any medication for it. However, after the recent test, he was prescribed Zyloric 100 mg, which he has used for two weeks. His fasting blood sugar level is 6.43. Please help.

You have not mentioned the current daily medications of the patient. Since your attending doctor prescribed Zyloric, I assume they have reviewed the medication regimen. Uric acid is a byproduct of digesting purine-rich foods. To reduce high blood uric acid levels, it is advisable to avoid purine-rich foods such as red meats, poultry, fish, organ meats, and shellfish. Staying well-hydrated, limiting alcohol and sugar intake can also be beneficial. The diabetic medication, Janumet 50/500, containing both sitagliptin and metformin, should not exacerbate or trigger gout. Given your well-controlled diabetic condition, it is recommended to continue your current medication.
