Hello doctor,

I am a 24-year-old female with a chief complaint of DIP and PIP pain in both hands. The DIP pain is more predominant. The pain scale varies and began directly after holding on to the grips of the treadmill for a little over an hour. I have never had pain when doing this before, but I am not sure if it is due to overuse or a combination of that and psoriatic arthritis. I have had psoriasis since I was 19 and it tends to be mild. I recently started birth control (about one month ago) and I know it has had an effect on my hormones as well as psoriasis. I am quite young though so I wanted an opinion on whether arthritis should even be considered. There is a slight warmth at the joints and a lot of stiffness. It slowly subsided during a couple of days following, and then the pain increased the next morning after I had “cracked” these joints the night before. Extremely minimal discomfort in my thumbs.

Psoriatic arthritis is seen in 15 to 30 percent of psoriasis patients. There are different types of psoriasis arthritis. One type predominantly affects DIP joints. The nature you are describing like warmth, swelling, and lots of stiffness points towards psoriatic arthritis. If your early morning stiffness is more than 30 minutes and your pain or stiffness decreases after activity then you should be evaluated for psoriatic arthritis, since you have psoriasis in your background history. Look for markers of inflammation in blood like ESR and CRP. Consult a Rheumatologist for a physical examination. DIP joints are commonly also involved in osteoarthritis but your age is very odd for osteoarthritis.
