Hello doctor,
I had surgery for my gynecomastia a few years back, and the results were satisfying. However, I have noticed that the incision scar on the right nipple has become slightly raised and prominent with time. Is this normal? How can I control and reverse it? Please help.

I went through your query and understood your concern.
It seems you have a keloid. It is due to excessive fibrous tissue, which can happen to anyone. The cause is genetic, or you have a tendency for hypertrophy of the scar. When the scar is raised and involves the surrounding normal tissue, we call it a keloid. It has not been confirmed in which patient it will develop. So, I suggest an intralesional steroid injection is the best treatment for you.
Some dermatologists use a combination of drugs (steroids and intralesional 5-Fluorouracil) for intralesional injection to see how it responds. I suggest you take an intralesional Triamcinolone injection at an interval of three weeks. The response should be observed after the first three injections before further injection schedules are planned. Other treatment modalities include pressure garments and silicone sheets, which can be used along with intralesional injection. Kindly consult a specialist, talk with them, and take medicines with their consent.

I will reach out to a local surgeon for the injection. However, I have a follow-up question regarding silicone sheets. Can you recommend any specific brand to be used? Should I apply the sheet directly to the keloid scar on the nipple? Please help.

I suggest you apply a silicon sheet directly to the scar. Any good brand will do. But change it after two months.
