What should be done in a person having kidney disease and blood in urine with high creatinine and protein levels?

Hello doctor,

My wife's blood reports, urine reports, and ultrasound show that there is kidney disease. The doctor says she is having blood in her urine and also a persistent cough. According to the test reports, her creatine and protein levels are high. Sometimes her BP is high.

I have gone through the reports (attachment removed to protect the patient's identity).

I can see a small stone at the end of the ureter and the size of it is slightly on the borderline for it to be expelled spontaneously on its own. It may or may not. But anyway I would suggest that you need to see a urologist and get that stone removed. It is called URSL (ureteroscopic laser lithotripsy) with DJ (double-J) stenting since it is increasing back pressure on her kidneys as you can see hydronephrosis on the ultrasound report. The sooner it is done, the better it would be. It is a simple procedure of a maximum of half an hour and can also be done as a daycare procedure that may not need prolonged hospitalization. Regarding her protein and creatinine levels in the urine, it is alright and not much bothersome in the presence of the USG (ultrasonography) finding.

The probable diagnosis is VUJ (vesicoureteric junction) calculus on the right side.

The investigation to be done is CT (computed tomography) KUB (kidneys, ureters, and bladder).

The treatment plan as of now would be to take lots of oral fluids and may not need medications. Also, she can drink good amounts of water with lemon.
