Hello doctor,

I have had a white spot on the foreskin of my penis for two years that is growing in size and becoming whiter. It is occasionally itchy, and the skin feels thin. However, there are no lesions, warts, redness, or irritation. I read online that it could be lichen sclerosus or vitiligo. Is it harmful, and does the lesion need treatment?

It is lichen sclerosus, an autoimmune condition that requires treatment. Initially, it is treated with a potent topical steroid. I suggest you apply Clobetasol Propionate 0.05 % ointment twice daily for 12 weeks. (Consult a specialist doctor, talk to him or her, and start the medicine with their consent). If it is still persistent, we need to start with the second line of treatment.

Thank you for the response.

I have used Clobetasol ointment for only three days, as I feared there could be skin thinning. Should I continue using it? I also thought it could be a fungal infection as I had fungus-like symptoms when the discoloration started.

A fungal infection causes sloughing of skin, whitish necrotic tissue, itching, redness, and pain. If a fungal infection occurs in this condition, you will require treatment for 14 days with Canesten cream. You must also avoid sex during this time.
