Hello doctor,

My mother is diabetic and also she takes medication for hypertension, cholesterol, and cardiac problem but for the past three months, she reduced the amount of food she consumes in a day and started to become weak. After a month she lost a lot of weight and developed psychotic symptoms then her doctors suspected that it might be infection or malaria so they gave her Meropenem, Cefitrax, and Vancomycin for the infection, and Artesunate, Arthimeter for malaria but nothing changed and they started giving her Olanzapine, Fluoxetine, CPZ, Imipramine, and Haloperidol for her psychotic symptoms and she started to become better even though she was still confused, the doctors cleared us to go home and we came to our home then she stopped eating so we took her to hospital again after five days of being discharged from the first hospital after that they inserted nasogastric tube and we are giving her food six times a day since then but her health is deteriorating and her liver function test, electrolytes, and kidney function test fluctuate. Sometimes it comes to the normal range and sometimes it increases or decreases for example she entered hypokalemia four times within two months.

Kindly suggest the treatment.

I can understand your concern but you have to understand that with these multiple problems going on, you alone will not be able to help her so it is advisable to seek medical attention right now and see a doctor who can be close to you and can support while taking care of so many problems for your mother taking care daily.

I suggest you follow a few instructions mentioned below.

  1. Blood sugar is important if it goes more than 200. An accurate amount of insulin or medicines have to be given daily.

  2. BP (blood pressure) monitoring has to be done.

  3. Try to change her scenery. If she is limited to bed then try to play some music around her.

  4. If she is mobile, then you can take her for walks or even ask some of your relatives and friends to visit her. This might change her mood and also bring little different environment.

  5. As there are many multiple health issues that are going on so I will advise you to see a doctor who can come for a home visit for these basic regular checkups for her. Keep a constant eye on her.

  6. If she is unable to eat by mouth properly, a doctor can also put a ryles tube (a tube that goes through your mouth to the esophagus and then to the stomach) it will provide the nutrition to your mother.
