Hello doctor,

My recent blood tests show high homocysteine and low ferritin, vitamin B12, folate, methylmalonic acid, iron, TIBC, and transferrin saturation are normal. My symptoms are muscle pain, tiredness, shortness of breath, and mild vertigo. I have a history of gastritis and constipation. Can you please interpret my tests?

  1. Your low ferritin suggests reduced iron stores. However, a normal transferrin saturation level indicates that iron transport in the bloodstream may still be adequate at present. The significance of this finding depends on your hemoglobin level, which would help determine whether iron deficiency anemia is also present.

  2. For shortness of breath, auscultation should be done, and cardiac history should be ruled out with an ECG (electrocardiogram).
  3. Mild vertigo, if constant, could be an inner ear infection for which a decongestant or short-term steroid course was prescribed. However, please consult a specialist, discuss with them, and start taking the medications with their consent.
