Hello doctor,

I am 32 years old, 5 feet 6 inches tall, and around 167 pounds. I am masturbating two to four times in the last seven years. For two years, I have had urine problems. Recently, I consulted a urologist and took an X-ray RGU and MCU. My report is enclosed herewith. There is no problem in the urethra and prostate. I doubt the pelvis nerves. So, is it necessary to take any pelvis nerve-related scan? Please help.

Please help me with a few details so we can get to the bottom of your concerns. Have you had any surgeries in the past? Do you have diabetes, hypertension, or any other illness? What problem do you face with urination? Is it a slow speed or urgent? Is there an increased frequency? Do you have to strain to pass urine, or do you feel incomplete bladder emptying? Have you had any other tests done except for the RGU (retrograde urethrogram) and MCU (micturating cystogram)? Are you sexually active or have kids? Do you have any trouble with the erection or ejaculation? I must say masturbating once or a maximum of twice a day might be considered acceptable, but four times is a bit too much. It would help if you allow yourself time to recuperate.

Thanks for the reply.

There was no surgery in the past. I am not yet married. I have no diabetes, hypertension, or other illness. Yes at times the urine passes slowly. Sometimes I feel pain and burning while passing urine. I strain to pass urine. I did not undergo any other test. I am not sexually active and have no kids. Sometimes I feel a problem with my erection. Whenever there is an erection I feel the urge to urinate. Please help.

I suggest you do a few tests and revert to me with the results: Urine routine and microscopy, semen culture and sensitivity, vitamin D, ultrasound of abdomen post-void residual urine, and uroflowmetry. The urine flow is not adequate at all. The normal speed or qmax should be around 25 for your age. Could you please do all the other tests and get back to me? Also, this issue with slow speed has no relation to masturbation unless you have been highly vigorous during masturbation and damaged the urethra.

I understand your concern.

No, you cannot damage your penile nerves by masturbation unless you are incredibly rough and bending your penis at awkward angles. Penile nerves run deep inside the pelvis. It is not possible to damage them externally. I will start you on medication and revise the medicine based on your test results: Tablet Alfoo (Alfuzosin) 10 mg once until the following review. Please continue to take this while you get the other tests done.
