Can Diane-35 permanently fix irregular periods in PCOS?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hi doctor,

My sister is 22 years old. She is suffering from irregular periods. She is taking Diane-35 to get regular periods. If she stops taking the tablet, then her periods will go missing.

What is the permanent solution for her? Additionally, she has a thyroid problem. She is under regular medical care for her thyroid. Please help.

Thank you.

Hi,

Welcome to icliniq.com.

As I can presume, your sister is having cystic ovaries with irregular menstruation, termed polycystic ovarian syndrome (PCOS). Here, the ovaries produce hormones in a disordered manner, and there is no production of eggs. Probably it can be due to obesity or hypothyroidism also. My question is whether she got tested for this by blood investigations and ultrasonography or took this pill empirically?

If not, then she needs a detailed hormonal test to diagnose the cause. If she is having PCOS, then the treatment starts with weight reduction as well as lifestyle modification, such as avoiding junk foods, engaging in exercises such as swimming and walking regularly, and avoiding a stressful life. You can ask her to start these things immediately. Continue the pill as well as thyroid medication for six cycles at least. Check her thyroid status once in three months.

Also, do serum LH (luteinizing hormone), FSH (follicle-stimulating hormone), prolactin, TSH (thyroid-stimulating hormone), T3, T4, testosterone, and DHEA (dehydroepiandrosterone). Also, ultrasonography of the whole abdomen has to be taken.

I hope this helps.

Please revert in case of further queries.

Thank you.
