Hello doctor,

I have MGUS, and my bone marrow biopsy was taken a couple of years ago. My test results are as follows: free kappa LT chains: 35.2 mg/L, free lambda LT chains: 40.5 mg/L, immunoglobulin results 3,559 mg/L.

Should I be worried?

I had MGUS 3 to 4 years ago, but I have had it for a decade after looking at my blood work reports. I have pain in the throat that comes and goes, and I have severe pain in the right lower side of my back that comes and goes. My bones ache badly all the time, and I am exhausted. When I did the PET scan three years ago, the doctor said a hot spot was located on the left side of my throat. But he did not go through the details.

I went through the reports (attachments removed to protect the patient's identity). The values are high, and other values such as platelets and white blood cells are normal, so nothing to worry, but you have symptoms, so you need proper treatment to control the disease. They mentioned that the hot spot is due to autoimmune illness only, and you have MGUS, an autoimmune disease. Autoimmune diseases are those in which our body develops antibodies against its own body. So it has to be controlled by giving symptomatic treatment. I would suggest you take tablet Calcium Vitamin D3 once a day before sleeping for ten days. And a thyroid function test will help us know if the hot spot is causing thyroid function impairment and thyroid hormone changes. So do thyroid function tests (TFT). Are you taking any medicine?

Thank you for the reply.

I have taken vitamin D tablets once a week for eight weeks, and I am supposed to take Metoprolol for blood pressure. But I am waiting on a refill.

Yes, Metoprolol is for high blood pressure that you can take and continue with your Vitamin D supplements.

But if bone pain does not stop, I suggest you add tablet Etoricoxib 90 mg once when you have pain in the back or bone.

It is anti-inflammatory, and it will help in healing as well.

And please do not worry.

Get a thyroid function test and send me the reports.
