Hi doctor,

My early morning urine output is very less and dark yellow even though I drink 8.4 fluid ounces of water before going to bed at 11 PM. After a few hours, I get normal urination. I urinate 10 to 12 times from the morning at 9 AM to the night at 11 PM. My CBC test is clear, and my USG KUB shows nothing abnormal. My urinalysis shows 4 to 6 RBC/hpf without protein. The other symptoms are frequent upper body fatigue, frequent bloating or cramping sensation in the right abdomen, foamy urine. But foam bubbles clear on flushing, and I have occasional leg pain. I have not experienced other symptoms like swelling around the eyes or in feet, no nausea or vomiting. My past medical history is that I had a 0.23 inch kidney stone on the right kidney detected last year, which was removed by ureteroscopy. Then I took K-Cit potassium citrate (0.5 fluid ounces in 3.38 fluid ounces of water per day) for the next six months till the beginning of this year. Now, my USG shows no stones. But I am very much concerned about other symptoms. Can you advise how to go about it?

What you have is microscopic hematuria. Previously, you have been treated for stones, and now you are stone-free. The risk of stone reoccurrence is fifty percent in ten years. To avoid reoccurrence, you should take preventive measures. One of the causes of microscopic hematuria will be stones or crystals. The other causes will be infections, kidney diseases, and cancers. For you, since your age is less than 40 years and a non-smoker, there is a minimal chance of having cancer. Since you got a history of stones before, then it is likely to be crystals causing it. To prevent reoccurrence, you need hydration of the body by taking 0.79 gallons a day, and if you are exercising, you should take 1.18 gallons. You will need blood tests of calcium, uric acid, phosphates, and magnesium and 24 hours urine collection for metabolic studies, including oxalates, citrates, calcium, uric acid, phosphate, and magnesium. The rest of the things with you are fine as kidney functions are fine, the urine shows no proteins, and the ultrasound shows no stones. Please do follow up with test results.
