Hi doctor,

My grandmother is 66 years old. She is having severe back pain near the spine and shoulder pain for the past two days. She used a painkiller but there is no relief. Please suggest.

Please mention the history of trauma. Please mention the exact location of pain whether it is located in the back or radiates to the leg. Please mention whether your grandmother has a history of tingling and numbness.

She has pain only in her back and shoulder. There is a sensation of tingling while moving, sitting, and standing.

As it is the first episode, it seems that it occurs due to back muscle spasm and inflammation. Tingling and numbness can occur due to nerve stretching and irritation. Your grandmother should avoid bending, weight lifting, and long traveling. She needs rest for some time. She can take tablet Dan MR (Chloroxazone with Diclofenac and Paracetamol) two times a day after taking food. She can take tablet Omeprazole 20 mg two times a day before food and take capsule Calcium 500 mg after taking food. She can apply Diclofenac gel.

Treatment plan:

Your grandmother can take tablet Dan MR (Chloroxazone with Diclofenac and Paracetamol). If it is not available, she can take them separately. She can take tablet Omeprazole 20 mg two times a day before meals and capsule Calcium 500 mg one time a day after taking food. She can use Diclofenac gel for local application. Follow these medicines for a week.

Regarding follow up:

Start taking medicines for a week. Have a follow-up after a week.

The pain present in the shoulder and neck has got reduced. But still, she has pain at the center of the back spine. She has tingling while standing up, sitting down, and lifting hands. She gets pain even while having a slight movement. Please suggest.

She can take tablet Zerodol P 4 mg (Aceclofenac with Paracetamol) two times a day after taking food and tablet Deflazacort 6 mg two times a day after taking food, capsule Omeprazole 20 mg two times a day before meals, and Calcium 500 mg after meals.
