Hello doctor,

I am having a lot of pain and burning in my chest. I had an ECG done about 20 days back which was normal. I have pain on the left side, (it is like on my breast) sometimes I feel it moves. I have been suffering from this since more than a year. Last year, I went through a complete evaluation and my ECG, 2D echo and TMT were normal. I was diagnosed with acidity but not on any medication now.

There are two possibilities in you, first is costochondritis (musculoskeletal) and the second possibility is of gastric reflux followed by fibroadenosis. Such a long duration of the pain and pain which increases on movements or pressing over the same site is usually costochondritis. Also, get your breast palpated once for any lumps (fibroadenosis), which can also cause pain if feeling pain is in the breast. You may continue Pantoprazole Domperidone combination (Pan DSR) for a week period, and if there is inadequate relief then get prescribed anti-inflammatory medicines from GP like Naproxen 250 mg twice a day along with Pantoprazole for five days. So, overall, the first possibility in you is of costochondritis and should have anti-inflammatory medicines like Naproxen.

So, there is nothing to worry about my cardiac health, right? Is a 20 day old ECG good enough? Do I need to repeat it?

Yes, it does not appear to be a matter of worry. If your pain appears to be of similar nature as that of previous pain but severe in intensity, then ECG is not needed. However, if the character of the pain is different from previous pains then better to get ECG done, for just to be sure. However, the description does not appear to be cardiac in nature. Also, cardiac pain will not continue more than 48 to 72 hours, and will not be affected by movements. Also, cardiac pain is associated with sweating, uneasiness, etc. Also, it is better you visit a local doctor and get yourself examined to be sure. All the best.
