Hello doctor,

I am a 28-year-old and I have some worries. I suffer from gastritis and also have myopic astigmatism. I have been experiencing some tremors in the past, which were on and off and sometimes worst when I drink alcohol. I have been off alcohol for three months now but I experience tremors with blurry vision. I experience pain in my chest during strenuous activity, which extends to my back. I have taken multivitamins for three weeks and I consulted with a cardiologist here who recommended Revital tablets and I started that yesterday. I notice cracking sounds in my knees when I climb up the stairs. There is a pain in my waist too. I have not been very active for about a year or more and I was diagnosed with aslo last month. I took four shots of Benzathine and Dexa injection. I am very worried and I am also experiencing some pain behind my ears, which extends to my neck. This pain is on and off and especially when I am traveling or in a car or active. The doctor prescribed Lumirelax and said I had no ear infection My heartbeat is sometimes fast. I really need to talk to a doctor. Kindly help.

Moreover, it seems that has caused concerns for you to include intermittent hand tremors, intermittent neck and back pain, and occipital headache. I have not been diagnosed with heart disease yet. The cardiologist only said I might have a shortage of vitamins and minerals. Some of the files are in another language so I had to use a translator. I am attaching all the various lab tests I have done. I will try to be clear. I have not been diagnosed with heart disease or anything yet. Two months ago I was diagnosed with UTI and I have received the treatment already. I was diagnosed with ASLO (400 IU/ml) and received injections for a month. Now, I have observed mild tremors and blurry vision. Lower back pain, neck pain which affects the ears (not consistently), and knee pain especially when I wake up in the morning. All these are mild discomfort and I am concerned about them. Besides these, I feel fine and function well. The conclusion shows that I have is hypochromic normocytic anemia, lymphopenia, and thrombocytopenia. What does that mean? Also, in one of the tests, a slight steatotic hepatomegaly with the increased caliber of the portal trunk is seen. The doctor told me it can be normal in some people but after that, I was given very strong antibiotics and vitamins. I need to know if I have a deficiency of iron, vitamin D, or B12 because I searched on the internet and saw that this might be the cause according to my test reports too.

I read your lab result (attachment removed to protect the patient's identity). You had a positive anti-streptolysin antibody (ASLO), urinary infection, and mild anemia with mild thrombocytopenia and lymphopenia. The ASLO (anti-streptolysin O) and your history of receiving Benzathine Penicillin were the cause that I supposed you have treated for rheumatic heart disease. I am sorry for this misunderstanding. Tests for hepatitis, HIV, and malaria were negative. In addition, evidence of fatty liver and gastritis was noted. In another lab test, you had low sodium, potassium, magnesium, and albumin. I did not see any test for screening of generalized inflammatory processes, thyroid function test, adrenal gland function test, and liver function test. These are important because tremors may be the result of dysfunction in any of those systems. Considering your complaints and your tremor, my recommendation is to request your family physician to check the following tests:

ESR (erythrocyte sedimentation rate), CRP (C-reactive protein), ANA (antinuclear antibody test), anti-ds-DNA, AST (aspartate transferase), ALT (alanine transaminase), ALP (alkaline phosphatase), bilirubin (total and direct), TSH (thyroid stimulating hormone), FBS (fasting blood sugar test), cortisol level and ACTH (adrenocorticotropic hormone). In addition, as I stated before, a short video of your tremor is very helpful. After conducting the mentioned tests and seeing your tremor, I can guide you if more evaluations such as whether brain imaging is necessary or not.

I was diagnosed with fatty liver two months ago but I will repeat the test as you recommend with the other tests. Also what test can be done for the pain in my knee, and feet after waking up in the morning or sitting for long, or cracking sound? If the test is included already then kindly let me know.

Fatty liver does not cause low albumin unless it is in severe forms. It is very odd that your albumin is low. I recommended liver function tests for this issue. If your liver works properly, the only reasons may be malnutrition, malabsorption, or excessive excretion of protein in the urine. Your urine test did not show excessive protein excretion. So, we should first evaluate your liver function. Morning joint pain, also known as morning stiffness, or pain after an inactive position like sitting, is a suggestive sign of a possible inflammatory disease. The tests for inflammatory screening that I suggested previously, are in this regard.
