Patient's Query

Hello doctor,

I am currently having a lot of popping in my neck in the C5 to C6 area. I have had pain for two months over the deltoid insertion after an incident with a blender. I had a spoon in the blender and accidentally hit the on button and the spoon I was holding hit the blade and it jarred my arm. It did not hurt at the time, but later on that same, the pain in the deltoid insertion started burning. Since that time, I am not unable to fully raise my right arm, the one that I was using, and I cannot reach behind my back any higher than my waistband. I also had been sleeping on two pillows and bought a new pillow at the same time so I am not sure which caused this. I have since then started doing neck stretches and sleeping on a flatter pillow and they are helping but I work on the computer all day and it is staying flared up when I am on the computer. Does this sound like shoulder impingement? I work for an orthopedic clinic, so that is why I am familiar with the wording but I do not know what I did and how to fix it. Currently, I am in between insurances so cannot have it checked right now. I am using Ibuprofen (Acetaminophen) or Tylenol (Paracetamol) and ice packs, they do help some but the mobility does not seem to be changing. When I raise my arm, which I can only get halfway, I believe is 90 degrees, my neck pops at C5 to C6. When I am using my mouse on the computer and move my arm from the right to the left side of the keyboard, it will pop too in that same area. My pain is not that bad and I take very minimal medications but the mobility is like my shoulder is frozen and now my shoulder has started to ache a little. Sorry to be so long but I am trying to give all the details.

Thank you.

Patient's Query

Hello doctor,

Thank you for the reply.

I would like to tell you that I am at my desk most of the day but have a lot of activity going up and downstairs where I live. I have 14 stairs from one floor to the next and I use them all day.

Thank you.

Hello,

Welcome back to icliniq.com.

Well, thank you for the detailed information. I have gone through your medical history and I feel your problem is a mixture of periarthritis shoulder and cervical spondylosis. Now, if we want to actually look into it, then ideally we should get an MRI (magnetic resonance imaging) of the cervical spine and shoulder region. My suggestion is to do conservative management at least for six to eight weeks before going for an MRI (magnetic resonance imaging). In conservative management, you need good anti-inflammatory medication along with muscle relaxants. You can go to a physiotherapist who will do IFT (interferential therapy) or TENS (transcutaneous electrical nerve stimulation) and teach you some exercises. But one suggestion from my side, take out separate time from your schedule and do walking for 45 minutes at a stretch daily. You must be doing a lot of stairs but that is done in breaks which is not beneficial, so start this. If you feel no relief in six to eight weeks, then go for an MRI (magnetic resonance imaging). I am sorry, my answer is compact, if you are not able to understand anything or need elaboration on something, then let me know.

Regards.
