Hi doctor,

I have some issues with my mental health I feel a voice inside my head, thinking always about the worst consequences, the worst things I can not even imagine. I can not control this. I try to forget it, but I am just failing again and again. This has happened before, but somehow I managed to overcome it because it was not severe. But this time this is severe. I feel like hating myself for those worst things coming to my mind, but believe me, I do not mean any of these. This problem is really bothering me, I can not enjoy my life, concentrate, or spend time with family as those awful things come into my mind. I feel helpless. Please help.

From your description, it appears to be ruminations. Kindly do a self-rating test for OCD (Obsessive-compulsive disorder), which you can easily find on the internet. If it is indicative of OCD, I suggest you consult a psychiatrist in person for a detailed evaluation and treatment. Treatment is available in the form of medication and psychotherapy. Besides certain other things that can help:

1. Practice relaxation techniques. Patterned breathing exercises like Sudarshan kriya (a form of breathing exercise) have been found to be especially helpful.

2. Log down any distressing thoughts, it will help ease associated anxiety and then try to put them down to logic.

3. Whenever a distressing thought comes to you, try to focus on relaxing imagery. Say to yourself that it is irrational and you will overcome it.

If these methods do not help, I suggest consulting a psychiatrist or psychotherapist. Remember, every problem has a solution; you just need to try solving it and help is just a step away.
