Hello doctor,

I am a 23-year-old male, weighing 121 pounds. When I was 17, I used to watch sexual content, and after a few months, I experienced my first nightfall. Subsequently, I had regular nightfalls, occurring around 10 times a month, which affected my studies. I sought treatment from a homeopathic doctor, and the prescribed medication reduced the nightfall frequency to four to five times a month. However, when I stopped taking the medicine, the nightfall returned, and I needed to resume the homeopathy medicine to control the frequency for the next two years. At the age of 19, I got a job and stopped thinking about the nightfalls, and they became normal without medicine. However, some other symptoms persisted, such as a slight burning feeling after my first-morning urine and a sensation of sticky water after urination. Additionally, I noticed more frequent trips to the toilet compared to others. Sometimes, when I was cycling, the pressure of sitting caused a sticky water sensation.

When others talk about sexual things in front of me, I experience the sticky water sensation, which I do not know how to describe in medical terms. Despite these symptoms, my urine test showed no infection. I am unmarried. Currently, I have refrained from watching any sexual content or engaging in any physical sexual activity. Over the past year, I have focused on building good habits, such as reading books, cutting back on social media, spending time with friends and family, and traveling. Despite these positive changes, I am still concerned about this issue and hesitant about marriage because of it. Please help.

I understand your concern. Wet dreams, also known as nocturnal emissions, are normal. They can be caused by increased erotic stimulus mentally, physically, or visually without sex or masturbation. At your age, testosterone production is good, and approximately 20,000 sperm are produced every minute, leading to significant semen production. It is natural for the body to release this semen either through masturbation or sex or during relaxed sleep.

To treat and manage this, you can reduce the frequency of erotic stimuli or masturbate or have sex as needed. Here are some ways to reduce the occurrence of wet dreams:

  1. Limit the intake of caffeinated beverages and foods after late afternoon, as caffeine can stimulate the brain and cause erotic dreams.

  2. Take a cold shower before bedtime to decrease excessive sensitivity in the genital region.

  3. Manage and control your stress levels, as stress can be a triggering factor for wet dreams.

  4. Avoid sleeping on your back or stomach, as these positions can cause sleep orgasms. Sleeping on your side is better.

  5. Exercise regularly to keep your body and mind relaxed, as excessive tension, anxiety, and body tiredness can contribute to wet dreams.

  6. Avoid watching or reading erotic content at night, as they can stimulate your body and mind and result in wet dreams during sleep.

Regarding your specific concerns:

  1. A burning feeling in the morning during the first urine is not related to nightfall. Drinking adequate water can help.
