I masturbate regularly. For the past three weeks, I have experienced numbness, pain, and a constant burning sensation in my penis. I also have frequent urination and difficulty telling when I need to pee. I recently had a urine test, and everything came back normal. What could be the issue, and is this condition permanent?

Thank you for your prompt reply.

Here are my answers:

  1. Are you diabetic or hypertensive? No.
  2. Have you had any surgeries in the past? No.
  3. How many times a week would you say you masturbate? About 5 times a week.
  4. Is it prone masturbation, or while sitting or standing? I usually do it while sitting, but sometimes I do it standing as well.
  5. How many times a day do you have to pass urine? Around 8 to 10 times now daily, compared to 3 times before.
  6. Does any urine leak before you can reach the toilet? No.
  7. Are you having normal erections? If I engage in activity for some time, I can achieve an erection, but it is painful.
  8. Can you please share your urine results with me? Yes.

Here is my advice for you to follow for the next week:

  1. Tablet Cranpac D 1 (cranberry extract and D-mannose) tablet in the morning and one tablet in the evening for seven days.
  2. Tablet Econorm 250 mg (probiotic) one tablet in the morning and one tablet in the evening for seven days.
  3. Tablet Mirago 50 mg (Mirabegron) one tablet in the morning for seven days.
  4. Tablet Chymoral Forte (trypsin and chymotrypsin) one tablet in the morning, one tablet in the afternoon, and one tablet in the evening for seven days.
  5. Capsule Urimax 0.4 mg (Tamsulosin) one capsule in the evening for seven days (Please note that this may cause lightheadedness on the first night, so be cautious if you wake up during the night. This side effect is likely to lessen from the second night onwards).

Thank you for the reply.

I do not have access to these medications as I am a student and do not have insurance. Could my diagnosis possibly be pudendal neuralgia or nerve damage, as it has been persisting for around three weeks? I am experiencing constant burning pain in my penis, scrotum, and pelvis. I have noticed it's worse when I am sitting or lying down. I have an ultrasound appointment in five days. I am feeling extremely scared and anxious.

No, you do not have pudendal neuralgia or nerve damage. What I do believe you have is a mixture of prostatitis and urethritis. I say prostatitis because it tends to flare up significantly when people remain seated for long durations. I forgot to ask earlier, have you had any unprotected sex in the last month or so, especially with a new partner? Unfortunately, I cannot get you the medication. You will either have to have your family send them to you from India (if that is legally possible) or you will need a prescription from your local general practitioner. Also, please remember to:

  1. Drink at least 101.44 fluid ounces of water per day
  2. Avoid tea and coffee for a week
  3. Have at least a glass of lemon juice a day for a week.
