I have been experiencing yellow urine for the past four to five days and slight soreness near my left testis. Please help.

I understand your concern.
Your symptoms point to a testicular infection called orchitis (inflammation of one or both testicles). In addition, your blood reports (attachment removed to protect patient's identity) show diabetes (a group of diseases that result in too much sugar in the blood).
Orchitis is common in people with diabetes. You will need to take an ultrasound and color doppler for the scrotum. That will confirm if the infection is present. If present, then a course of antibiotics will be needed. For symptomatic relief, take a tablet of Paracetamol (Acetaminophen) 500 mg whenever needed. I suggest you consult an endocrinologist for diabetes.

The cold sensation on your penis is also a part of the infection. Because your urine result does not indicate any urine infection, infection of other body regions, such as the testes and urethra, should be investigated.
The two most important types of care you require are infection control and diabetes management. As previously indicated, you should get a scan for confirmation and consult a urologist (a specialist specializing in urinary tract disorders) for a physical examination.
