Hello doctor,

I am a 32-year-old male and weigh 308 kg. I always have a high blood pressure of 150/90 mmHg while meeting the doctor. The doctor always thought it was due to my anxiety. The echocardiogram results showed normal left ventricular size and function, LVEF 60 to 65 %, grade 1 diastolic dysfunction, and mild LVH. For my age, should I worry about my result? Also, can this be reversed? And how much is the life expectancy? Currently, I am on Toprol 25 mg for past one month for PVC.

I have checked the attached report (attachment removed to protect patient identity). The echocardiography report shows mild enlargement of the left ventricle (the chamber responsible for pumping the blood to the body) due to high pressure. It is a mild enlargement and controlling blood pressure strictly will improve the long-term prognosis.

Diastolic dysfunction is decreased amount of blood filling the heart due to the enlarged heart muscle; it is mild, you should not worry. Strict blood pressure control, i.e., 130/80 mmHg is required. I suggest that you have a digitalized device at home. Toprol (Metoprolol) 25 mg may not be strong enough. I suggest changing it to other medication with more effective and better results.

Reversibility and life expectancy depends on how you control the blood pressure.

Yes, for sure controlling blood pressure strictly will improve the prognosis greatly and you will live a normal life. On the other hand, not controlling the blood pressure will cause serious consequences. I suggest Ramipril 1.25 mg once daily in the morning (we can upgrade it to 2.5 mg once daily if target blood pressure is not achieved within one week).
