Can an increased follicle size in a woman undergoing ovulation induction indicate pregnancy?

Hi doctor,

I am a 27-year-old female and I am undergoing ovulation induction. I had tablet Letrozole from day two to day seven. My USG details are as follows:

  1. Day nine - Endomet: 5.9 mm; right ovarian follicle: 19.5, 20 mm; left ovarian follicle: 15.8 mm.

  2. Day 10 - Endomet: 7 mm; right ovarian follicle: 20.5, 21 mm; left ovarian follicle: 17 mm.

  3. On day 10, I did HCG (Human chorionic gonadotropin).

  4. USG on Day 13 - Endomet: 10 mm; right ovarian follicle: 24, 29.5 mm; left ovarian follicle: 17 mm and the impression is that the follicular size has increased.

My doctor said that a rupture had occurred and prescribed Progesterone to be taken from day 13 to day 26. But, why has the follicle size increased? Is it a possibility of pregnancy?

I went through your reports (attachment removed to protect the patient’s identity), and feel that the follicle has not yet ruptured, since its size has increased and there is no fluid in POD (pouch of Douglas). Therefore, I suggest you repeat the follicular study tomorrow to check whether it has ruptured or not. I hope you are having intercourse during this time. Continue taking folic acid tablets and keep yourself hydrated by drinking at least three liters of water a day. Also, include green leafy vegetables and fruits in your diet.
