Hello doctor,

I have been undergoing regular vigorous sexual intercourse and frequent masturbation. A few weeks ago, I felt symptoms like a painful erection, tender penis, erectile dysfunction not sustained, slight discoloration of the penis, and abnormal blood flow. The doctor advised giving up masturbation for a couple of weeks as it may be traumatizing or fatiguing. My STI test was negative, and undergoing a UTI test for the prostrate check. I am concerned about the tenderness felt in the upper shaft. On pressing softly, I feel a shooting pain in my muscles. There is dark discoloring, bruising, and slight inflammation. How to reduce tenderness? Kindly help.

Thank you for your reply. No, I just took the tablet Ibuprofen. I have refrained from all activity, and the shaft still feels swollen. I do not think the tablet Ibuprofen has helped. I am unsure if there is a topical ointment to apply.

The causes of the swollen penis could be due to allergic reactions, sexual injury, and balanitis. Most cases of balanitis result from an overgrowth of fungus or bacteria. Allergic reactions are primarily due to condoms, spermicides, and lubricants used during sex. You are having features of UTI (urinary tract infection); it could be due to urethritis. Urethritis also leads to painful penile spasms. I would suggest you take the below medicines for a week.

Tablet Fluconazole 50 mg once a day after food for one week.

Tablet Amoxyclav (Amoxicillin and Clavulanic acid) 625 mg, one tablet thrice a day after food for five days.

Consult your specialist doctor, discuss with them and take medications with their consent. If symptoms do not subside in a week, you need to undergo an ultrasound, color doppler of the penis, and fasting blood sugar test.

I can not recall an injury or trauma. I did use a condom, and the oral activity on me was rigorous; if I remember, the top of the shaft felt a bit tired and numb. Oil was used on the penis. In that area of the top of the shaft, there is a bump or raise, and could this be balanitis? What is balanitis? I cannot attach a video as this is the best way to see it. Not sure if visible by picture and would need a visual appointment. Do you provide a prescription?

Balanitis is a type of skin irritation that develops on the head of the penis. Rigorous oral sex will predispose to infection. A bump on the head of the penis is suggestive of balanitis. If your symptoms do not decrease with the medication, kindly consult a local urologist for a local examination.
