Hi doctor,

I am currently on medications for allergic bronchial asthma since last year, having on and off flare-ups of recurrent cold infections. I took oral steroids for a short period (five to seven days Wysolone 10 mg once daily). I consulted an ENT specialist yesterday, and the doctor asked me after checking my X-ray taken two months ago if I have chest pain, and I said yes, sometimes I have it. What made him think so? Please guide me if there is something to worry about as my cardiac markers are also high (attaching latest reports). No history of COVID or vaccination done. I am taking the current medications, and the doctor has now prescribed me to take Hetrazan 100 mg twice daily for 14 days for long-standing allergies (IgE levels have always been high for above ten years now, mostly 700 to 800). And have gross DNS. I do not have a high AEC count. Can I take it safely, or can it cause any anaphylactic reactions (a bit afraid because of allergic reactions sometimes)? Is it used for this in normal practice also (attaching allergy reports too)? Is this the right line of treatment?

I am a 38-year-old female. My BP is 110/70, my weight is 152 to 154 lbs, and my SpO2 is 98 to 99. At the age of 15 to 16, I used to have digestive issues of constipation and excess gas. Then since 21 years, I started having allergic issues that included urticaria rashes (sometimes and allergic rhinitis, which over the years converted into bronchitis and asthma). I had iron deficiency anemia many times, then had viral keratitis post lasik 11 years ago, also developed GERD and piles. Then I developed panniculitis or abdominal TB post my cesarean seven years ago (attaching reports) (took ATT twice). I got vulva swelling from the next day of cesarean six years ago that lasted for a good period of time. I developed herpes in the arm during ATT and later on sacroiliitis. I was also developing warts on my body and foot corns for the last two to three years. I do not know if all these have any connection.

I am taking medicines for bronchial asthma (attaching prescription) and sacroiliitis, Saaz and Pregabalin. Also started taking absolut 3 g Multivitamin for last three days. Current symptoms for the last one month are swelling in the left upper eyelid and tongue sometimes, bad breath, sometimes toothache and blurred vision, and a bit of asthma flare-up, with discomfort in the nose, constipation, and gas for the last one month. Kindly give your opinion.

I went through all your blood investigation reports done ten months ago, nine months ago, and last month (attachments removed to protect the patient's identity).

1) Your blood levels of C-reactive protein and lactate dehydrogenase (inflammatory marker) are constantly above normal limits. Serum iron percentage and transferrin saturation is low and, in correlation with your complete blood count reports, indicates iron deficiency anemia. Your fasting blood sugar and HbA1c (hemoglobin A1c) are suggestive of impaired glucose tolerance or pre-diabetes. Your blood total IgE (immunoglobulin) is also high.

2) High total IgE is probably due to bronchial asthma, which you suffer from. A high level of IgE is also seen in many parasitic infections.

3) You are currently prescribed Hetrazan 100 mg (Diethylcarbamazine), which is an antihelminthic drug and not an anti-allergic.

4) I will strongly advise you to avoid short-period oral steroids like Wysolone 10 mg without a doctor's advice each time. Taking short-term steroids on and off will decrease your cell-mediated immunity and will put you at risk of suffering from many infections, including parasitic infections. Taking on and off steroids will put you at risk of developing diabetes.

5) I will suggest you get an additional stool test for ova any cyst. Sometimes, your IgE level can get elevated in parasitic infections like hookworm and roundworm infection. These infections can cause associated anemia. In these conditions, their eggs are passed in the stool sample. Do give three samples on three consecutive days for stool ova cyst examination and occult blood.

6) I Have seen your specific allergy panel, which are all negative. Still, I would like to advise you to reduce intake of milk, eggs, fish, vegetables like brinjal and citrus fruits like lemon and orange. Avoid cold food items too.

7) To take anti-parasitic medication (tablet Albendazole 400 mg one tablet once a day for three days, preferably after a fat-rich food meal. Fat helps in better Albendazole activity) after giving a sample for stool examination. Do not start on medication before giving the stool sample for the ova cyst test.

8) I Would like to advise you on an Iron supplement as you are suffering from associated iron deficiency anemia. Start with this medication after stool ova cyst, occult blood test, and after three days Albendazole treatment. Do remember this. Do not take iron supplements with calcium-containing food like milk.

9) There is probably no connection between these and body warts and foot corn.

10) No need to take Absolut 3G Multivitamin tablet for now.

11) During this treatment, I will not advise taking Saaz (Sulfasalazine). You can continue the anti asthmatics as per the doctor's advice who prescribed it.

Thank you.

1) I am not finding any laboratory nearby my place for conducting both of these stool tests. If you can, please provide the details.

2) Because of the dietary insufficiency and abdominal-related issues. I started Absolut 3G after consulting with my doctor as I feel lethargic all the time and digestive issues. So, if you can please guide me if it because of the test I should stop or for some other reason. My doctor gave me an Iron supplement that has the same content available in this also (that is, Ferrous Fumarate 30 mg to be taken for one month).

3) Can I take Hetrazan safely (I had to start it from today) as of for now till I get my tests done? Does this medicine cause any severe allergic reaction?

4) With all my history provided, do you think it can be the correct line of treatment with Hetrazan (never taken this medicine though I have tried many anti-allergic and antibiotics). I have never taken steroids of my own except once in the month three months ago, that too after trying to reach the doctor, but because of this pandemic, the doctor was not able to consult.

1) Laboratories, where you have done your previous tests, will do stool tests too.

2) You can go ahead with Absolut 3G and Iron medication containing Ferrous Fumarate. In such a case, no need to take the Iron supplement suggested by me.

3) I do not recommend Hetrazan for now in this situation. We will decide after your stool examination report to know if you are suffering from any parasitic infections.

4) Your doctor would have suggested this medication based on his differential diagnosis. Hetrazan will help in killing the parasites inside your intestine.

I got my stool test results done yesterday. I am attaching the reports. Please check the reports. Though I took Hetrazan twice yesterday, I felt relaxed without having shortness of breath. I do not even feel like taking inhalers. I had normal bowel movements today after a month. What is the reason for this? Can I continue taking Hetrazan? Can I use Systane ultra and Iflomax eye ointment? Can I take them with Hetrazan? Can I take multivitamins along with it? I am afraid of drug interactions and I want to know whether my reports are indicating something different.

I acknowledge your stool occult blood test result and ova or cyst examination result. There are no abnormalities detected in the stool examination. You can follow the advice provided by your physician. You can continue Hetrazan, multivitamin, and iron supplements. You can take Hetrazan in the morning and night as recommended. Take multivitamin and iron supplements in the afternoon after lunch. You can use Systane ultra ophthalmic solution (Polyethylene glycol and Propylene glycol) and Iflomax ointment (Carbomer and Vitamin A) advised by your previous doctor. Take a balanced nutritious diet. Do regular walking and exercise. Do breathing exercise which are recommended for asthma such as deep breathing exercise and pranayama. Do meditation and yoga regularly. Have a cup of green tea twice a day. Have a strict watch over your blood sugar. Avoid certain food items as I previously advised like avoidance of milk, egg, fish, vegetables like brinjal and citrus fruits like lemon and orange for few months. Be calm, stress free and relaxed all the time. Being in the pandemic time, follow the personal protective measures while going out like wearing mask, follow hand hygiene and social distancing. Do I need to do other stool tests on the second and third day or any other tests to rule out parasitic infections? I started taking Hetrazan yesterday after providing my sample. According to you, what is the diagnosis of my condition with the reports provided? If I am on the correct line of treatment, will this become a habit-forming drug? Do I need to take them again and again in the future? I have high HS-Crp levels. Does this get increased in other conditions? Do I need a cardiac evaluation? Do I have any features of sarcoidosis or fungal infection? I am neither covid positive till now and have not been vaccinated. Is it safe to get vaccinated? Which vaccination shows the least allergic reaction? My husband has eventration of the left diaphragm since birth. He does not have any other issues except sometimes where he needs to take a long breath on exertion. Can he safely take vaccination? I am attaching his reports. Apologies for repeating my queries. 1. Do the other two tests for confirmation.

2. It seems to be high IgE. It probably occurs due to some parasitic etiology. Rarely, it can be due to Hyper IgE syndrome.

3. You are going on appropriate treatment. Do not be worried.

4. There is no need to be worried about heart for now. Repeat these tests after six months and have a review. Do fasting blood sugar, postprandial blood sugar, HbA1c (glycated hemoglobin), complete urine analysis, complete blood count with peripheral smear, Total IgE (immunoglobulin E), CRP (C-reactive protein), Hs-CRP (high sensitivity C-reactive protein), ECG (electrocardiogram), chest X-Ray, echocardiogram, pulmonary function test, and stool examination for ova cyst and stool occult blood test.

5. It does not look like sarcoidosis or fungal infection. It looks like parasitic etiology probably.

6. All the vaccines against COVID 19 are safe. You can choose any vaccine of your choices like Covishield or Covaxin.

7. Both you and your husband can take the vaccine safely. Remember to get both doses of vaccines without fail at a recommended interval. Even after vaccination, follow hand hygiene, social distancing, and wear a mask.

Do follow my other advice. Avoid taking unnecessary steroid medication. Follow exercise advice and food advice. Brush your teeth twice a day and use chlorhexidine mouth wash after brushing twice a day to overcome halitosis (bad breath). You can do respiratory exercise with a respirometer. With reference to my last communication, I wanted to ask something. Till now I have taken a course of Hetrazan for eight days. Initially, I felt extremely relaxed for three to four days. After four to five days, I started having breathlessness with some headache and body ache without fever. I had some dental pain and swelling in one of my eyes on the fourth day. I also have left facial swelling and it got subsided the next day. My eye floaters and swelling are coming on and off till now. On some parts, I feel relaxed in my stomach-related issues of constipation, pain below the ears, and allergy. Sometimes, I have uncontrollable asthma.

This humid season makes my asthma symptoms worse. How can I manage my asthma properly? Is this medicine work? I am also taking Ceftum 500 mg twice daily. It causes allergy, itching, nausea, and dry cough and so I stopped taking it yesterday. In my last week, I also stopped taking Abphylline and Montair LC. I continue taking hetrazan, Flomist nasal spray, Mucinac twice daily, and Pan 40. Are these flare-ups common while on this treatment? How can I counter them?

On the first day when I was taking Hetrazan treatment along with Budamate inhalers, tremors started and bronchospasm was felt that I did not felt before. So I do not know how to use them and I do not want to use inhalers again when I am getting a bit relaxed with this treatment. Can I use Abphylline N twice a day for five days alternatively or with the above-mentioned medicines of Pan, Hetrazan, Flomist instead of an inhaler? or say whether I can counter the asthma flare-ups or inhalers has to be taken? Today is the ninth day of taking Hetrazan. Can I also take Zerodol SP on sos basis for dental pain and headaches? It helps me to get relief from them. Please guide. What is better to control this?

I understand your query. The tooth pain might be linked to headaches and swelling of the eyes. You may be suffering from a dental problem like tooth decay. You must visit a nearby dentist and get it treated or else it will lead to some complications. Do not take Zerodol. Go to a dentist and get your dental treatment done. If you are not comfortable with Hetrazan, you can stop taking it with the advice of the physician who prescribed it. You can use Abphylline N (Acebrophylline with Acetylcysteine) and Montair LC (Levocetirizine with Montelukast) for a short duration to control your asthma. There are certain things that control asthma. Do regular breathing exercises with a respirometer, do pranayama and other gentle breathing exercises, do mild exercises. Keep your room and bedsheets dust-free and pollen-free. Do steam inhalation. Take water boiled with herbs like holy basil. Take frequent warm water sips. In this humid climate, you can have steam inhalation for 5 to 10 minutes daily but not too hot steam. Take care.
