Hi doctor,

I am a 24-year-old male. I have an issue with similar symptoms of a UTI. And both times, I have gone to urgent care for it. I have got tested and treated for UIT and chlamydia. Both times I have tested negative. My symptoms are white milky discharge and pain in the penis while urinating and passing stools.

For your symptoms of urethral discharge and pain in passing urine with increased frequency and urgency in the long term, treated with antibiotics and tested negative for STI twice, the most probable diagnosis is chronic prostatitis. Other possibilities, like bladder stones, are less likely. Chronic prostatitis can be bacterial or without bacteria, but both types respond well to antibiotics. For the diagnosis to be confirmed, please do the some tests which are, ultrasound KUB (kidney, ureter, and bladder), and semen for culture. Please follow up with the results to enable me to select your medications according to the culture results. Please note treatment for chronic prostatitis can take up to four to six weeks with antibiotics.

Does this make sense looking back at my diet? It seems the times I have had spicy food, alcohol, and caffeine, it has been far worse. I do not have a PCP yet, unfortunately, and my appointment with one is a few weeks away. What is the best thing I can do now until I can get proper testing and medication?

If you do not have access to a laboratory or ultrasound, you can use tablet Ciprofloxacin 500 mg twice daily for four weeks. Then after the treatment is complete, you can do the tests. Treatment for this problem (chronic prostatitis) is sometimes challenging and requires follow-up.
