Hello doctor,
My partner has very tight tendons on the sides of his penis that cause pain with erection. He has attempted to stretch out the tendons, but they go right back, and he is concerned about damaging them. He gets relief from the pain when he presses down on the penis. He has also heard a popping noise. He saw a urologist today and was told there was nothing wrong. Any suggestions?

During vigorous sexual activity sometimes there is a partial tear of the superficial layer of tunica of corpora cavernosa leading to such description. If he is able to achieve the erection with no ecchymosis, redness or blackening, it is fine. Ask him to share a picture of an erect penis in two directions.

He is reluctant to send a photo. There is no discoloration and the penis is straight. He has had this discomfort for many years. It appears to be tightness in the area of the inguinal ligament. When erect he can locate a ligament and actually pull on it to stretch it out. He also has experienced pain for several years in his lower ribs (also on the right side). He is taking physical therapy. The doctor says there is excess cartilage at end of floating ribs. Could this be causing the groin problem? Possibly a pinched nerve in the rib area?

It is good that there is no discoloration, erection is straight. Chronic pain for a long time is complained by many young patients without any long term consequence. Groin pain is also related but transient. Recent exposure to sexual activity also causes groin pain. Rib tip excess cartilage is not caused because of pain. But muscles going to groin originate from rib tips which may get pulled during vigorous activity. So nothing to worry, all this is due to new activity involving the generalized body contraction like sexual intercourse and will settle with time. Tablet Zerodol-MR (Aceclofenac + Tizanidine) two times a day, and tablet Rantac (Ranitidine) once a day for five days.
