Hello doctor,

I felt itching on the top part of my penis. Later I developed a red spot-like ulcer that was spreading gradually. I started taking Fluconazole 100 mg once a day and applying candid B cream. But no improvement. Please help.

I read your query and understand your concern. I have seen your query and concern and examined your picture (attachment removed to protect the patient’s identity). It seems you have balanitis. Balanitis is inflammation of the glans penis. Balanitis involving the foreskin and prepuce is termed balanoposthitis. Predisposing factors include poor hygiene and overwashing, use of over-the-counter medications, and non-retraction of the foreskin. It may be associated with diabetes and candida fungal infections. Investigations- blood sugar KOH mount for candidiasis.

Treatment:

Gentle retraction of the foreskin daily and soak in warm water to clean the penis and foreskin. You can use a combination of Fusidic acid and Clotrimazole twice daily for one month. If balanitis does not respond to topical therapy then you can take Ketoconazole 200 mg twice daily for two weeks after consulting your GP. I hope I have cleared all your doubts.
