Hi doctor,

I have a pimple type bump on the left side in my mouth on the lower gums for the last ten days. There is no pain in the pimple but it pains while eating at the teeth present in the back which are used for chewing. I take medicines for BP, brain, and cholesterol. Please help.

I am taking Olmy D 20 and Cilacar M 10/25 BP medicines, along with that Nootropil 800 mg.

BP medications are fine. Do you have any memory loss episodes as Nootropil (Piracetam) is generally administered for Alzheimer's disease? I am very sorry about that. As I requested earlier, an image of the painful tooth was essential and helpful. I would ask you to upload that to walk you through the management that would be best for you. If possible, do upload the image.

Please try,

- Capsule Sporidex (Cephalexin) 500 mg one capsule twice daily for five days.

- Tablet Metronidazole 400 mg one tablet twice daily for five days.

- Tablet Zerodol Sp (Aceclofenac and Paracetamol) one tablet twice daily for five days.

- Capsule Pan D (Pantoprazole) one capsule before meal for five days.

- Betadine mouthwash (Povidone-iodine) or gargle in lukewarm water.

- Avoid sticky or sweet food as much as possible.

Start the medication, they are safe and nothing to worry.

Do not apply any external heat. Brush twice gently, rinse whenever you eat.

Kindly upload an image of the offending painful tooth, use a flash to capture the image. Kindly focus on the pimple area as that would confirm the abscess formation and will also determine the treatment needed to resolve the issue once and for all.

Once I receive the image, I will assist immediately and walk you through the treatment needed to resolve the issue once and for all.

I went through the attached images (attachment removed to protect patient identity). I will walk you through the management needed to resolve the issue permanently.

  • Root stump of the lower right first molar and third molar is noted.
  • Caries in lower second molar.
  • Sinus tract noted, suggesting periapical abscess due to the decayed tooth.
  • The root stumps (lower right first molar and third molar) need extraction as they might form abscess again later on.
  • The lower right second molar needs root canal treatment followed by a fixed bridge involving the lower right second premolar as a pillar.

So, I would suggest starting the medication as I prescribed as soon as possible. Once the drug gets over, get the root stump extracted, followed by a lower right second molar's root canal. Please do not waste further time as more the infection, the less likely the chances for a successful root canal.
