Can oral antifungals, along with topical antifungal cream, help overcome pityrosporum folliculitis?

Patient's Query

Hello doctor,

I have a rash on my body that initially started on my legs. It has now spread to my back, arms, hand, and face. The rash appeared as small white bumps a few days ago. I went to the doctor, and they diagnosed it as an allergic reaction. I was prescribed antihistamine and steroid tablets, but unfortunately, the rash has worsened during this time. Additionally, I am taking a contraceptive pill. What could this be? Please help.

Thank you.

Hello,

Welcome to icliniq.com.

I understand your concern. I have reviewed the images (attachments removed to protect patient's identity). This could potentially be a viral exanthem. Could you please let me know if it is itchy? When did the rash initially start? Have you experienced any recent illness? You can continue using only antihistamines, and I would also recommend using a soothing lotion.

If you have any other specific queries, please do not hesitate to ask or call anytime.

Thank you.

Patient's Query

Thank you doctor for the reply,

I have not been unwell at all; this just came out of the blue. It is also very itchy. I noticed small white raised bumps on my legs over the weekend, but I did not think much of it. As of now, they have become inflamed and itchy, and it appears to be spreading.

Hello,

Welcome back to icliniq.com.

Another possibility could be pityrosporum folliculitis, which manifests as itchy, red bumps at various body sites. This condition is caused by an overgrowth of fungus. You might want to discuss this potential diagnosis with your general practitioner. Ask if they could also consider it and prescribe oral antifungals, in addition to a topical antifungal cream.

Thank you.

I hope your queries are resolved and any further queries are welcome.
