Why are my glucose and cholesterol levels high in pregnancy?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hi doctor,

I have a lot of stress in my life. I have lab test reports that are not normal, such as high glucose, cholesterol, low urea nitrogen, etc. I have attached documentation for you to see what I am trying to say. I am currently pregnant, and my due date is in the next seven months. Please recommend follow-up lab tests to confirm if I have prediabetes or any other illness? I can also take lab tests immediately, or if you believe I have a diagnosis based on these lab results, I would like to know. Also, I need information about some eating habits to change. I have been mainly walking, not doing much exercise. Currently, I am taking Prenatals and no additional vitamins (I am low on iron in the last two pregnancies). Also, I am hoping to get everything to the optimal range on my tests. If you have any other recommendations to prevent anything from getting worse, I would appreciate it. Kindly help.

Hello,

Welcome to icliniq.com.

According to your labs, your fasting sugar is high, but the hemoglobin A1c (HbA1c) test, the predictor of the average sugar over the last three months, is normal. I suggest you start a diabetic diet that limits your sugar intake and has food with a low glycemic index, along with brisk walking for at least 30 minutes a day for a minimum of five days a week. I also suggest you get the glucose challenge test done with a load of oral glucose to confirm prediabetes and get fasting glucose levels checked at regular intervals. Cholesterol levels are high, but currently, you do not need medicines for that in pregnancy.

Kindly revert in case of further queries.

Patient's Query

Hi doctor,

Thank you for the reply.

Should I plan to get a test for cholesterol after the pregnancy? And will I need medication? Kindly help.

Hi,

Welcome back to icliniq.com.

I suggest you repeat the cholesterol and other tests after pregnancy. Your current levels are not so high that you would need medicines. It is managed with a good diet and exercise.

Thank you.

Patient's Query

Thank you doctor,

Hey, so these are the follow-up labs. Would you be able to please point me in the right direction?

Hello, welcome back to icliniq.com. Your sugar levels are normal, so there is no risk of even prediabetes. Make sure you eat well, as hypoglycemia is dangerous too. Also, your CRP (C-reactive protein) levels are raised, which could be due to an infection recently. Did you have a fever, cough, diarrhea? Hepatitis A is also abnormal, so if you have GI (gastrointestinal) upset, that could be the likely reason. It is generally self-resolving.

Patient's Query

Thank you doctor,

I always get diarrhea once or twice a month unless I get sick, it will be worse, but once I go to the bathroom or throw up, I usually feel better immediately unless I get more bile out. I haven't been sick recently. Should I do any follow-up tests? Also, what about the sedimentation rate and t3? Are those concerning?

Hi, Welcome back to icliniq.com. T3 (triiodothyronine) is normally low in pregnancy. So you need not worry about it. The sedimentation rate is high due to infection. You can repeat the CRP and sedimentation rate after a month. Also, you need to undergo screening for group B streptococcus infection, and it is a vaginal swab.

Patient's Query

Thank you doctor,

Ok, I have had bacterial vaginosis twice before. Should I test for that? I was thinking about doing a yeast infection test at home. Also, should I get flu and hepatitis A vaccine? Is there anything I could do to speed up hepatitis A? I am getting std results next week. Should my three-year-old son get tested for anything? Will I could expose him too? Does Hepatitis A have its healing process?

Hi, Welcome back to icliniq.com. Yes, you can get a yeast test at home and a BV (bacterial vaginosis) test. You can take the flu vaccine. Hepatitis A is not needed now. If your son does not have any symptoms, then no need for testing.

Patient's Query

Hi doctor,

Thank you.

1) Is there anything I can do to prevent infection from possibly harming the baby?

2) Is there anything I can do to prevent it from getting worse?

3) Are there any vitamins you recommend for me to take other than prenatal?

4) Should I get an annual exam even though I am getting prenatal care?

5) Is it ok if I am not gaining a lot of weight? I am doing a lot of exercise and eating a lot healthier than I was. Actually, I was overweight before I got pregnant.

Thank you.

Hi,

Welcome back to icliniq.com.

No, the prenatal vitamins should be sufficient with prenatal care. Yes, weight gain depends on your BMI (body mass index). If your BMI prepregnancy was high, then advised weight gain is lower.
