Patient's Query

Hello doctor,

I am 27 years old with a height of 5'9" and a weight of 93 pounds. My health is perfect. But I have a problem with my sex life. Whenever we have sex I do not last more than 10 to 15 seconds so help me with this problem.

Thank you.

Hello,

Welcome to icliniq.com.

I have read your query and understand your concern. The condition you are suffering from is called premature ejaculation. There can be several causes of premature ejaculation (PE). Broadly they are divided into four types:

1. Due to medical conditions: This includes causes like hormonal changes, injury, and side effects of certain medications.

2. Due to other psychiatric conditions: Such as anxiety, depression, and addiction to smoking tobacco or alcohol.

3. Natural causes: Like having sexual intercourse after a very long time. People have noticed that frequent intercourse improves ejaculation time.

4. Functional premature ejaculation: No known cause.

As per your description, I am assuming that you are otherwise medically and mentally fit without any addiction. So I think you suffer from functional premature ejaculation. Now about the treatment part. There are two kinds of treatment for this type of PE. One is psychological, the other is medicinal. It is your wish which treatment you want to take first as both are equally effective. If one fails we can try another and still there is no complete improvement then we can go for a combined. Psychological treatment consists of M and J techniques and others. But for learning these techniques you have to have face-to-face or at-least verbal communication with the doctor. If you wish to try this technique first you can set up an appointment with your local psychiatrist or you can set up a video or phone consultation (personally I would recommend video) with me on this site.

Medicinal treatment: If you wish to try this first you can start with the tablet Paroxetine CR (Piperidine and Benzodioxole) 12.5 mg half at night for seven days and then one at night for the next seven days. Follow up after 14 days so that I can decide on the future course of the treatment depending on your response to medication. Remember the medicine can take four to six days to start its effect. I hope this helps you.

Thank you.

Patient's Query

Hello doctor,

I have tried the medicine till today but I found no result for that. Can you suggest something else?

Thank you.

Answered by Dr. Hitesh Sharma

Hello

Welcome back to iciniq.com.

Do not worry. I had given a very low dose as I cannot start with a full dosage. Please increase the dose to tablet Paroxetine CR 12.5 mg 1.5 HS (night) for seven days and then to two HS (night) for the next seven days. So after 14 days, you should be at least 30 to 50% alright. I hope you are talking 12.5 mg one HS (at night) till today without any break in treatment. I hope this helps you.

Thank you.
