Hi doctor,

I had pelvic pain, discomfort, and pressure while urinating and sitting. I have done a transrectal ultrasound scan (TRUS) and uroflowmetry. But I am still having backache and pain while urinating. My concern is if there is any problem with the prostate like prostatitis or prostate cancer.

Let me first start off by saying that this is not prostate cancer. Prostate cancer only ever occurs in men after the age of 50 to 55. So please take that out of your mind.

The symptoms you described suggest chronic prostatitis which is quite common in your age group. Please help me with a few details so that I can assist you better:

  1. Are you diabetic or hypertensive?
  2. Have you had any surgeries in the past?
  3. Have you ever had blood in your urine?
  4. Are you sexually active or masturbating regularly?
  5. Does it hurt when you ejaculate?
  6. Was a semen and urine culture test done for you?

Hi doctor,

Thank you for the reply.

I experience a sensation of something catching in the prostate area each time I urinate, which persists throughout the day. Despite undergoing a normal TRUS (Transrectal Ultrasound) and uroflowmetry, as recommended by a local doctor, my discomfort related to the prostate gland persists, hindering my day-to-day activities. I feel pain and discomfort, particularly when sitting.

Should I undergo MRI (Magnetic Resonance Imaging) for a comprehensive evaluation?

Please help me.

Your symptoms align with typical prostatitis symptoms, making an MRI unnecessary. I recommend the following tests, and based on the results, I will suggest appropriate medication:

1. Semen culture and sensitivity.

2. Serum PSA (prostate-specific antigen).

3. Urine culture and sensitivity.

Additionally, here are some general recommendations you should strictly adhere to:

  1. Avoid prolonged sitting; aim to get up every 1 to 1.5 hours and walk around for 2 to 3 minutes.
  2. Ensure adequate hydration by drinking at least three liters of water daily.
  3. Maintain regular ejaculation, either through intercourse or masturbation, at least twice a week.
