Hello doctor.

My 46-year-old wife has had pain, stiffness, swelling, and limited hand mobility during the past few weeks. She has no additional health conditions and is not taking any medications. It is important to rule out inflammatory causes of pain, swelling, and restricted mobility in the hand. The pain usually settles down on its own, but if it lasts more than six weeks and the morning symptoms last more than half an hour, it is most likely due to an inflammatory cause. I would also like to know whether there is any swelling in the joints of the hand, as well as any tenderness or redness. To identify the causes we may need to do a few tests such as X-rays and blood indicators such as ESR (erythrocyte sedimentation rate), CRP (C-reactive protein), RA factor (rheumatoid factors), and anti-CCP (cyclic citrullinated peptide). First-line therapies include physiotherapy and anti-inflammatory medications. Are there any other symptoms, such as exhaustion or muscular discomfort, that appear to be caused by vitamin D insufficiency and may need to be addressed as well.
