Can a 69-year-old have Klinefelter syndrome with RA?

Hello doctor,

I'm a 69-year-old male with a long history of various health issues, including recurrent infections such as systemic Candida, valley fever, and multiple fungal infections. I’ve also been diagnosed with rheumatoid arthritis (RA), systemic lupus erythematosus (SLE), and have tested positive for mixed connective tissue disease (MCTD). I’ve experienced Raynaud’s phenomenon several times as well.

Physically, I’ve always had a more feminine body structure, including gynecomastia, wider hips, and a generally softer build. I also had a significantly delayed onset of puberty, didn’t start shaving until around age 25, have long limbs and small testicles, and was never able to muscle up despite years of weight training.

Three years ago, I had a mild heart attack, but my coronary vessels were found to be unobstructed, and my heart function is reportedly normal. I was on medications for three years but am currently only taking daily Aspirin.

At present, I’m dealing with severe joint pain , especially in my hands, fingers, and the big toe joints on both feet. I also recently developed what seemed like a cold that progressed into bronchitis, with pain on my right side. My doctor has prescribed a five-day course of antibiotics.

I’ve never been tested for Klinefelter syndrome (XXY), which could be contributing to my health issues. Several doctors have brought it up in the past, but haven’t followed through with testing.

I’m concerned that I may not be receiving treatment for the root cause of my health problems. What would you recommend as the next step?

Recurrent infections involving different types of organisms, including fungal infections, may suggest a significant immune deficiency. This could be due to long-term steroid use for your mixed connective tissue disease (MCTD) or may be related to other conditions, such as diabetes.

Could you please clarify the following:

  1. Are you currently taking any steroid medications or immunosuppressants for MCTD?

  2. Have you been diagnosed with diabetes?

It's also possible that your immune system may be compromised due to the underlying autoimmune condition itself. Please share your most recent complete blood count (CBC) report for a more detailed assessment.

You should perform karyotyping to evaluate for Klinefelter syndrome (a genetic condition in which you have an extra X chromosome), especially since it has been previously suspected.
