Hello doctor,

I am a 21-year-old female. I am currently on birth control only. I went to the emergency last night because I wanted to get tested for an STI since I have been with a new sexual partner. The smell of my discharge is not normal, and I have been having bladder pains and a burning sensation while peeing and even while sitting still. I also woke up three days back with swollen tonsils and white spots at the back of my throat. I took a urine sample, blood test, vaginal swab, and throat swab yesterday but they said it could take several days to get the results.

In the past 10 hours approximately, my bladder and vaginal pain have gotten worse, and I run to the restroom to pee very frequently. I noticed blood in my urine a few hours back. I have had a UTI previously, but I have never had a sexually transmitted infection or disease. I have taken antibiotics for a UTI, but I am unsure whether it is a UTI or something else. Please help.

In case of unprotected sex, one needs to think of sexually transmitted infections and do the respective tests like the following:

  1. TPHA (Treponema pallidum hemagglutination).

  2. HPV (human papillomavirus) DNA PCR (polymerase chain reaction) test.

  3. Herpes simplex virus (HSV) one and two IgG (immunoglobulin G).

  4. Herpes simplex virus (HSV) one and two IgM (immunoglobulin M) serum.

  5. Chlamydia trachomatis IgG.

  6. HIV (human immunodeficiency virus) one and two antibodies, screening test.

  7. HbsAg (Hepatitis B surface antigen).

  8. Urine complete.

If not, it could be a simple bacterial infection for which I suggest you use the ointment T-Bact (Mupirocin) locally and take a tablet of Doxy (Doxycycline) 100 mg twice a day for five days.

If there is itching, then you need to think of a fungal infection for which you can use the Beclometasone plus Clotrimazole (Candid B) ointment locally and for itching you can take the tablet Cetirizine hydrochloride (Cetzin) once at night.Make sure you wash your genitalia twice a day in the morning and at night. Use normal water and do not use cold or hot water. Do not use a soap containing heavy pH. Your case mostly looks like Chlamydia or bacterial infection for which Doxycycline will be beneficial. In case of pain, you can take the tablet Aceclofenac plus Paracetamol plus Serratiopeptidase (Zerodol- SP).
