Hello doctor,

I received oral sex from a sex worker last month. I had an itching sensation in my penis head and also two rashes on my right hand. Almost in two to three weeks, the rash (reddish, small circle of 1 cm diameter, no itching) disappeared. Slowly itching reduced but not completely. May I know what can be the reason?

I read your query and understand your concern.

The chances of transmission of HIV (human immunodeficiency virus) by single unprotected oral sex is almost nil unless there are bleeding wounds in the oral cavity. It is unlikely as you would have noticed it.

There are some STDs (sexually transmitted diseases) like herpes simplex virus which can be transmitted by skin-to-skin contact or kissing or as STDs or oral sex.

It is not clear about the genital lesions. It may be due to infection or STDs or allergy.

The hand lesions need not be related to STDs. It may be due to fungal infection or eczema etc.

I suggest you take an STDs panel test (including HIV antibodies, HBSAGg, HCV antibodies, VDRL, HSV 1 and 2 IgM, and IgG antibodies) after three months of last exposure to be relieved of anxiety totally and conclusive results.

Hello doctor,

I underwent two HIV tests last month: One was HIV I Ag (P24) and HIV I and II IgG AB (ECLIA) test, and the other was an antibody test. Both tests came with a negative result. Both these tests are done after two years I had unprotected sex. So is this result conclusive considering that unprotected sex incident?

After those tests, this is the first incident of unprotected oral sex I had. Given the symptoms I mentioned earlier, do you see any possibility of HIV? I am sure their gum was not bleeding.

Additionally, are these symptoms related to any other specific STD?

I am experiencing itching on the skin-overlapped area of the penis head, as well as occasionally at the top of the penis head. I also noticed a small painless white blister (approximately 2 mm in length) that appeared slowly over two weeks on the penis head, and it disappeared after another week.

The HIV antibodies tests done after 2 years of exposure are conclusive for the said exposure.

The chances of transmission of HIV (human immunodeficiency virus) by mentioned current exposure appear to be nil.

The penile lesions mentioned may be due to irritation or friction or allergies or fungal infection or herpes infection. Nothing can be suggested without proper pictures. You can get back with clear photos of the lesion if present now.

If symptoms persist or worsen consult a specialist.

Thank you for the reply.

The blister disappeared the day before yesterday. Please find attached the picture of the penile. A whitish mark is visible now (approximately 2 to 3 mm diameter), circled in green around it.

I have gone through the photos (attachments removed to protect the patient's identity).

It has healed, for now, and so cannot make out the lesion. It may or may not be related to STDs.

I suggest you follow a few instructions mentioned below.

  1. Wear loose cotton undergarments.

  2. See that there is adequate ventilation.

  3. Maintain hygiene.

  4. Do not overclean the area.

  5. Clean the area with warm water twice daily.

  6. Do not use harsh soaps or irritants.

  7. You can use mild soaps or cleansers once to clean.

Hello doctor,

Thank you for the reply. Unfortunately I missed to take the picture when the blister was formed. But, I can explain. It was somewhat like a small (approx 2-3mm long, not circle shape) white white blister. It did not get burst or form as an ulcer. Just vanished off, now even no mark. No pain was there due to this blister. Iam concerned what would cause it.

Thank you for the follow up query . In my opinion this lesion may not be due to STD (sexually transmitted disease) .It may be due to irritation or friction or dryness or injury.

Same symptoms don't mean you have the same problem. Consult a doctor now!
