Patient's Query

Hello doctor,

I have right shoulder pain on certain movements. I had a right shoulder injury during racket sports. Please review my MRI images.

Thank you.

Hello,

Welcome to icliniq.com.

I have reviewed your attached reports (attachments removed to protect the patient's identity). I can see a large amount of fluid collection around the shoulder joint. That is caused by focal complete tears and retraction of 2 of the 4 tendons that make up the rotator cuff of the shoulder. The affected two are called the supraspinatus & infraspinatus, and the tear is about 3 cm away from their attachment to the humerus. The rest of the muscles, tendons, and shoulder joint capsules and cartilages are normal. So, it is quite a significant injury there, and you need to immediately consult an orthopedic or arthroscopic surgeon for further steps.

Thank you.

Regarding follow up

Consult an orthopedic or arthroscopic surgeon.

Patient's Query

Hello doctor,

Is the tear completely away from the humerus, or is it a tear leaving some tendon material on the bone?

Thank you.

Hello,

Welcome back to icliniq.com.

I understand it is distressing to discuss this, but I will try to be objective. As I mentioned earlier, there is a full-thickness complete tear of the supraspinatus and infraspinatus, as well as a tear of what is called the rotator interval. The tear leaves minimal or no remnants on the humerus. The torn tendons are retracted approximately 1.5 cm away from the humerus. However, I cannot see any fatty degeneration in the torn edges yet. And as I said earlier, there is some fluid collection around the joint, which is quite obvious at this stage.

I hope the above information helps.

Best wishes.

Thank you.
