Patient's Query

Hi doctor,

I have noticed a change in one of my skin tags, particularly in my groin area. It has become larger and round, and I am currently experiencing pain in that specific skin tag. I will be sending a picture for reference. What steps can I take to address this? Is it possible to remove it at home while traveling, or would it be advisable to seek immediate attention at a dermatologist's clinic? The affected area is in my leg in the groin region.

Hi,

Welcome to icliniq.com. I read your query and understand your concern.

I do understand your concern.

It seems to be an acrochordon (skin tag). A differential diagnosis includes wart or verrucae. Potential causes may involve insulin resistance, type 2 diabetes, chronic friction, obesity/overweight, or a possible infection. You could consider applying Podophyllotoxin cream (0.5 percent solution or 0.15 percent cream) twice daily for three days, followed by a four-day gap, repeating this cycle for three cycles. Alternatively, 5 percent Imiquimod cream could be applied three times per week every other night for a maximum of 16 weeks. If you prefer faster results, surgical methods like cauterization, excision under local anesthesia, cryocautery, electrosurgery, or ligation with a sterile thread to cut off blood supply can be explored, followed by necessary antiseptic measures. A few additional questions: Since when did it first appear? Are you a known case of diabetes? Please provide more details.

Looking forward to your response in order to help you.

Thank you.

Patient's Query

Hi doctor,

Thank you for the reply.

The skin tag has been present, and there is now an additional one. Previously, I had insulin resistance and pre-diabetes, and I was overweight. However, I have since lost 15 kilograms and no longer have diabetes, with an HbA1c level of 5.8. My current height is 5 feet 7 inches, and my weight is now 89 kilograms, down from the previous 103 kilograms.

Considering this, should I aim to reduce weight? Additionally, can I attempt to remove the skin tag myself, or would it be advisable to consult a dermatologist for the procedure?

Hi,

Welcome back to icliniq.com.

Electrosurgery is recommended for the removal. Additionally, it is advisable to continue weight reduction. Seeking the opinion of a dietitian could be beneficial for personalized guidance.
