Hi doctor,
I am a 38-year-old male. I am overweight. For the past 10 days, I feel tiresome. I have been taking tablet Nurokind-LC once daily in the morning and capsule Adliv Forte morning and night for the past four days.
Now, I am feeling better, but not 100 %. I suspect UTI, but have not tested yet. I have asthma since childhood. Initially, I used Foracort 200 inhaler, and now I am using a nebulizer with Asthalin respirator solution.
My wheezing is in control now. My lifestyle over the last few years was not good. I missed sleep and slept whenever I had to.
Please suggest ways to get better with or without tablets.

I understand your concern
Our body is created in a manner that we should work for eight hours, rest for eight hours, and sleep for eight hours. When we change this pattern, it may lead to many diseases like migraine, hypertension, diabetes, obesity, and others.
Please answer my above questions. It will help me find out why you are not sleeping well. Kindly refer to relaxation exercises and breathing exercises. Do it for 20 minutes before going to bed.
If your sleep does not become regular we can start with sleeping pills, which will greatly improve your sleep. My opinion is thatwhen you sleep well, your increased weight and tiredness will go away. Anyway you are just 6.6 lbs overweight. You can reduce it easily by going fast walking 40 minutes a day in the morning.
Nurokind-LC (Methylcobalamine) is usually given for cramps occurring at night. If you have that, you can continue with Nurokind LC. Better health tablet is Zincovit. I have tried that with many patients and found it is one of the best drugs for tiredness.
There is no need to take Adliv; Zincovit is more than enough.
Eating a fruit as a whole everydaevery dayyou all the vitamins you need and a good amount of fiber. Eat one fruit every day.
About UTI, since when have you suffering from pain during urination?
Please explain the symptoms, so that we can analyze and prescribe any test (if needed) and treatment.

Thank you for your thoughtful approach. Yes, I used to work various shifts and mostly the night shift in the past three years. I was working on more projects and more clients, and so I compromised with my sleeping hours.
Yes, I used to think too much day and night, and it disturbs my sleep pattern. I have been resting for the past three weeks. I recovered 90 % now, except for a mild tiredness when I try to concentrate heavily on any single issue.
I took a routine urine test and attached the report. I did not have any symptoms of UTI now. I just stopped both tablets, Nurokind-LC and Adliv Forte, after your reply. Now I feel that my recovery is getting faster.
I do not drink or smoke. I do not have any digestive problems or pain in my right lower rib area. I have been suffering from wheezing since I was a year old till now. I would like to know any advice for long-term prevention.

According to your urine test (attachment removed to protect patient identity), only a few extra pus cells are present, which poses no problem at all.
Night duty will spoil our health. We will get accustomed to one shift for some time and get some sleep after duty. But they will change the shift before we get adjusted to it. It gives a lot of trouble.
So, if you are going to do shifts again, please ask them to give you monthly shifts (regular night duty for a month and day duty the next month), so that the shifts will not cause any health problems.
You can take Zincovit tablets at morning every day for a month for your tiredness.
Regarding wheezing, start with the tablet Montair-LC (Montelukast Sodium). One tablet before sleeping for two months. It will prevent further episodes of asthma. If your symptoms occur once a month or rarely, then this tablet is not necessary.
In that rare case, you can take the tablet Septilin, which is an Ayurvedic tablet. Take one tablet at night daily. It will increase your immunity and will prevent asthma.

There is no doubt that day duty is the best. We are made up for that only. Since you have started your own setup, it is very good.
There are two kinds of people. One wakes up early in the morning, like 6 or 7 AM, and goes to sleep by 10 PM. Another typeof person who is comfortable waking up by 11 AM and goes to sleep by 2 to 3 AM. Both are normal only.
I think you are type 1. So, daytime work is better. Night duty is always harmful. Please find a relaxation exercise and do it at night. You will get good sleep. Once you start sleeping well, all your problems will fade away.
Also, start doing pranayama. Close the right side of the nose and take a deep breath using the left nostril. Hold your breath for a few seconds. Now open the right side and close the left. Exhale via the right side. Again, start inhaling on their right side. Proper practice will make you do that easily. Do pranayama for five minutes three times a day and before sleep.
Again, there are different timings of sleep. Some people feel better if they sleep for eight hours a day. Some need only five hours. You have to find out the timing that works for you and follow the same timing. For example, if you sleep for six hours and feel fresh the next morning, then it is the correct time. But if you feel tired, you may need more time.
Never have hot drinks like coffee or tea at night. Similarly, please avoid spicy masala food at night. Both will impair sleep.

With regards to pranayama effort, sometimes either one side of the nose is partially or fully blocked. So, I get a little bit discouraged whenever I try to do this exercise as suggested by some of my college seniors when I was a UG Student. Please let me know if there is anything I can do about it to rectify that issue.
Often, my sleep used to get disturbed either due to wheezing or due to other issues, such as the phone. Now I have learned to switch off all those gadgets three hours before bedtime. After using Montair-LC, I am not getting disturbed much except for a slight discomfort in breathing when I wake up.
I was using Asthalin respiratory solution ina nebulizer from the age of 28 to 37 and moved to Foracort inhaler 200 for almost a year back. At present, I am using only Montair-LC every night before sleep and Asthalin respiratory solution without diluting for a few seconds whenever I feel difficulty in breathing.
I took a blood test just to see how things are with my general health. I am attaching my CBC (complete blood count), LFT (liver function test), and urine routine test reports here for your review. I am 99% alright, no,w and my weight is 74 kg now. I am not feeling energetic to go for a morning tennis, which I have started recently, to have some fun outside of work.

Regarding pranayama, do it whenever your nose is not congested. If nose blockage is present, you cannot do it.
Also, I suggest lung exercises to increase your tidal volume. This will increase exercise tolerance. So, you will not feel breathless or tired when playing tennis. Just buy some balloons and blow 10 balloons a day. However much you do is fine.
You can start tablet Roflair (Roflumilast) 500 mcg at night. It works for 24 hours and reduces wheezing. It is like Deriphylline (Theophylline), which dilates air passages. Slowly, you can stop Foracort when you use Montair and Roflair.
Tennis is a game that requires a lot of energy and stamina. So start playing lightly for a month and gradually increase speed.

Yesterday, I had a wheezing attack after longtime. It was a gradual discomfort started from evening. I stopped taking Montair for few days. Also for the past ten days, I am coughing regularly in the morning. I am getting light yellowish color phlegm regularly but no difficulty other than that. The chest congestion is a bit high now and I feel there is a lot of phlegm around my chest. I am a bit concerned about using Montair regularly. Yesterday night, I took Betamethasone 0.5 mg one at 8 PM and one at 2 PM with Asthalin respirator solution in nebulizer and my wheezing was controlled. Please advice.

I am not using Foracort now. Because I do not feel good with the medicine like earlier. I just keep Asthalin inhaler in my car and a few Montair tablets for emergency when I go out in two wheeler or car. When I feel slight breathing difficulty, I am just inhaling one or two breaths of Asthalin respirator solution without dilution.
I am not going for any sporting activity yet as I feel little weak when I do something physically. Thought of continuing sports after two months. I am just taking rest at home now for past two months doing mild stuff at home and engaging in mental work for about three to four hours per day. Once I stopped Montair after a day or two, I am getting discomfort. So I got scared whether my body is going to get addicted to this medicine. Your response makes me feel better.

The main thing is that the wheezing is usually seasonal in many patients. Like autumn or in rainy seasons. So during that time Montair-LC is good.

Thank you for the good explanation and right reasoning about Montair. I understand your point here. I took Montair about 30 tablets till now in the past two months. Will continue Montair for one more month and then will review from there as per your advice.
Earlier, I was playing badminton (shuttlecock) in the night-time around 8 to 10 PM for few months. It was too tough for me as we played only doubles and it requires swift movements consistently. I use to get one episode of wheezing almost every day which gets controlled when I take two puffs of Foracort 200 with around few breaths break. If I play for another 2 to 3 hours even I do not get any wheezing the rest of the day.
That was my routine for about 3 months and in general most things were in control that time, then my health became so weak. So, I stopped playing badminton and tried tennis once or twice a week for about one month. After a month, I was bedridden with tiredness for almost 10 to 20 days and that is where I started to interact with you. Now I stopped all these and will try other things like cricket at home/walking/swimming etc as you suggested .Till I get a good health I will not go to racquet sports again.
I had fits once when I was around 20 years old and took Tablet Neptoin for two years and it got cured and never had fits again. Will it get triggered from my medication?

Hello Sir, Sorry for replying so late. I was terribly busy with classes since morning. Also i had to give a speech in a meeting today. For loose motion, you should take black coffee without sugar frequently. Also there may be a powder available in nearby provisional stores. Arrowroot powder. You can make kanji using that (just mix with water and boil). That will also reduce the loose stools. You have to take antibiotics or the diarrhea will be present for many days. The following tablets can be taken. T. Zenflox-OZ (Ofloxacin + Ornidazole) 1-0-1 after food for 3 days T. Eldoper (Loperamide) 1-0-1 after food for one day only If you also have abdominal pain or vomiting sensation please also take T. Pansec DSR (Pantoprazole + Domperidone) 1-0-1 before food for three days Electral powder 21g - two packets. Mix one packet with one litre of lukewarm water and drink in 12 hours (tonight to tomorrow morning). Open another packet and mix in one litre tomorrow morning and drink in another 12 hours. Hydration is more important. Drink plenty of fluids like home made juices. All my wishes for early recovery.

My diarrhoea is in control now ( followed your coffee advice and it seems working very well, yesterday I missed out on black coffee and had diarrhoea yesterday badly, but today it's fine ) No other symptoms as of now , everything is alright now. Do I need to start with those tablets for diarrhea ? Please let me know . Thanks.
Hahaha. Ok. If the symptoms comes back, take tablets. Otherwise no problem. That antibiotic I told, if we start that you have to finish three day course. So if you think you are getting well, no need for tablets. But if you have diarrhea today at least 2 times then pls start tablets. Regards.

Hello sir, Thanks for your wishes. I wonder why this diarrhea didn't stop. Usually it goes away with three day course. Another thing that causes diarrhea is mild gastritis - acidity. If you are feeling like passing stools immediately after taking food and didn't have at other times, then it may be due to gastritis. Last time I prescribed pansec DSR. That is a great drug for acidity. If you have motion immediately after food, this tablet is enough for next five days. Along with that you have to strictly control oily and spicy food. Nothing else is needed. But if you are passing loose stools not related to time of food intake, then it is due to infection only. Have you took zenflox oz for three days regularly? Even if you took please take it for another three days. Even after that diarrhea still persists, we go for motion routine test. Stop with black coffee sir. Too much of it may cause gastritis. Arrowroot kanji is good only. If you like that take it once or twice a day for two to three days. Please also take probiotic like T. Prepro or T. Lacto-B thrice a day for next three days alongwith antibiotic. It is good fungus, which stops loose motion. Summary If it is gastritis - pansec DSR twice a day before food for five days is enough If it is not due to gastritis 1. Antibiotic 2. Pro biotic for three days 3. If still persists till Saturday, motion routine test.

Hi Doctor,I took zenflox-200 ( mistakenly given by medical store and I also missed checking it ) for three days, and T Eldoper (1-0-1) for 1 day . Both tabs after food. May be this is a little lower in power than Zenflox OZ ? I neither had abdominal pain nor vomitting sensation . So did not had T Pansec . Had electral mixed with lukewarm water for two 12 hours . I was eating a bit spicy.Btw, I am taking one to two puffs of Foracart 200 inhaler in day time when needed as my body is responding well to it nowadays and continuing on Montair in night. My wheezing is in good control now both in day and night. Also had home made oily vada today and that may have aggravated loose motion, along with spicy gravy. Not going to toilet immediately after taking food. The time is random. So I guess I need to take Zenflox OZ and any 1 Pro biotic for next three days along with Electral 2 more packets plus avoiding oily/spicy food. Is this the right drugs for my condition ? Hi Doctor,Hope you are doing good. This is a follow up after taking around 30 days of Montair LC for my wheezing . Stopped it now and not feeling too troublesome . I still have this minor breathing difficulty which gets controlled if I take a puff of Asthalin Inhaler or foracart 200 or Asthalin nebuliser for few seconds. My body is responding well to all of these drugs a lot better than six months ago. In lifestyle changes, I'm trying to reduce spicy food, avoided tea/ coffee for past few days. Activity side, I'm doing mild household activities plus an occasional 10 to 30 minutes mild badminton or soft ball street cricket with kids around. Just thought of updating you for your review and further advice if any on drugs / lifestyle . Thanks a lot for your timely help when in trouble . Thank.
Hello sir, My apologies for answering so late. Great to hear that you were doing good sir. To escape from frequent asthma attacks, you have to increase your tidal volume of lungs. By blowing balloon twice a day you can increase your lung volume and episodes of wheezing may come down. Also refer internet for exercises for asthma. Continue septilin regularly which improves the immune system.

Hello sir, Asthalin is short acting and it is a bronchodilator. So when you have acute wheezing, Asthalin will be ok. Forocort is steroid and is used to prevent occurence of wheezing episodes. So, when your wheezing is controlled and if you want to prevent attacks, use forocort. My suggestion is, Use Asthalin only if you have a wheezing episode. Use Forocort now for few days. hello sir, its called status asthmaticus. it cannot be controlled at home. but there is one way. but i dont know how much feasible it is. Enquire local surgical shop (where u bought nebulizer) or search in net about oxygen cylinders. you can buy one and keep at home. They are saying that breathing oxygen everyday for 10 minutes is great for health. When you have an big attack, put nebulizer solution and connect the tube with cylinder and turn on the cylinder in full speed. That s what we give in ICU. also Betnesol or Dexamac tablet can help. These are all according to my experiences with patients. I dont know about practical problems in doing this at home.

Thanks Doctor, After 3 days of tablets, I am improving very well, and almost never had any symptom of having even small difficulty of breathing in the past few days. hopefully I can start work from today. Please advise. Oxygen is a good idea. I am researching on that to keep a small handy oxygen or the usual one in the hospital at home for emergency help.My Dad had these kind of attacks too many times and we have seen our regular nurse in the hospital injecting Deriphyline + betnesol in nerve and it calms down in 10 minutes 99 percent if we take him to hospital within 15-20 minutes. we usually come home after 30 minutes in the midnight without admitting him in the hospital most times.So with that background, now, this has happened for me 2 times in 2 years, may be after a stress / loss of sleep etc. Irrespective of reasons, I am starting to think about prevention / managing it better . My wife knows to take blood, as a lab technician she worked in a blood testing lab, but she has never worked as a nurse. My question is, if she gets trained as a nurse in some hospital for few months/years will she be able to do the vain injection for me at emergency times. in that case, we will be able to buy some time and go to hospital with more assurance than big tension/fear. Please let me know if this a feasible option. I asked this with the local hospital nurse, she said its difficult to get nerve at those times even for experienced nurse, so its not feasible. But still thought of asking you. Can injecting betnasol / Deriphylline in body help at those times ? Please take your time to reply. No hurry for this. As I am discussing long term things and not urgent issues. Thanks.
Its great that you are getting well sir. Since your wife was a lab technician, she can give IV injections. Instead of drawing blood, just pushing drugs. If she ties a torniquet, and you pump your wrist, vein will be clearly seen and she can inject. Just ask her to train one day in injection room in a hospital. Inj. Deriphylline and Betnesol are the commonest injections we use. I am using it for 10 years and they are the first line of choice even now for acute ashtmatic attacks. So I suggest strongly that whenever you have an acute attack, You can use your wife's help in getting an injection. There is no harm in that. If the vein is not available, then go to a doctor

What's your thoughts about Ayurvedic medicines ? Can I try that simultaneously with allopathic drugs like foracart inhaler / montair OrTry that once I stop taking English medicines ? Hello sir, Of course ayurvedic medicine is good, but I did a detailed search but couldn't find that it can be given with allopathic medicines. Nobody knows because nobody ever did a research of combining this too and studying side effects. I will do one thing. First i will try Ayurvedic medicinesin one of my patients for two months who is having mild allergy/asthma. If it really benefits, then Will try along with allopathic medicines and will tell you in two to three months. Since you have moderate to severe asthma if not on allopathic drugs, you cannot stop our medicines and just start them. I suggest you to consult an ayurvedic physician that whether you can start allopathy and start with these drugs. Tell him that you are on nebulizers and Montair. Since they would have seen much more patients, they may tell whether stopping allopathy and starting ayurveda has much help or not. Usually they treat chronic patients. Also you can do one thing. Start Ayurvedic medicines along with our medicines and we will do some blood tests every week whether it is causing any damage or not. Take care Regards

Thanks doctor for your frank answers :) . It helps a lot. Even I thought the same, that you are doing some research on that. I dont mind waiting for clarity on this with some factual evidence . Agree with your thinking in this. I will try to get back to you after 2-3 months on this Ayurvedic medicine issue. But as an information for you , I already used Ayurvedic medicine for about 2 months now,, ( but stopped after status asthamaticus recently )My doubt is that, Allopathic medicines work temporarily stopping phlegm obstruction in airways. so allopathic medicines kind of make them stick wherever they are.On the other hand, Ayurvedic medicine, being ayurvedic, and it may work on the opposite, to loosen it up and get out of the body, so,, I had a doubt, whether my recent status asthmaticus was due to this... I did spit a decent amount of phlegm once I was in hospital and on the betnesol,deriphyllene injection for about 5 minutes while I had high difficulty breathing. then both wheezing came down.. and spitting of phlegm also stopped.. So I got a bit confused about using Ayurvedic medicine or Homeo medicines like Blatta Orientalis Q which some say.. would cure asthma completely.. ( for bulky individuals)On Blatta, I tried it along with Foracart inhaler / Asthalin Nebulizers.. no big complications.. Wheezing definetely came down from daily wheezing difficulty to about weekly once kind of . So it worked well for me.. but whenever I get stuck.. with a severe attack I use to fear.. and stop that for a few weeks.. till I become a bit normal. without frequent breathing difficulties. I got stuck about a year back with status asthamaticus and stopped using after that with the doubts of combining both allopathy and homeo together in same time.I am trying to find the long term solution.. so asking a lots of questions may be a little out of your specialty ( allopathy),, but,, being a doctor and an south indian. I guess you understand these issues better.. and my concerns.. and I hope you know the abilities of Ayurveda / Homeo etc.. on specific chronic illness .. for permanent cure. I hope you dont mind these questions.. I dont want immediate answers to this. All I am trying is to some how find a long term solution for preventing one more status asthmaticus in my life.. Please stop ayurvedic medicine and other drug and please do the following blood tests Blood Urea Serum Creatinine Liver Function tests Urine- Microalbuminuria You should not have taken them. Please stop. Nobody knows whether combining these drugs are safe or not. Because no study has been done regarding that yet. I will tell you an alternative solution, Which may be permanent. I dont know whether it will work for or not. But no harm in trying. There is one injection called Inj. Histoglob which i started using recently. It is an allergy breaker. It contains many stuffs like mite, house dust, pollen, etc., which causes allergy and asthma to many people. By giving this injection repeatedly, our body may become bored of this allergens and won't create wheezing or allergic attacks. Please try. No Side effects (May be some pain and redness in injection site). This injection should be given in buttocks Inj. Histoglob 1 vial Dosing info: First week-Three injections (alternate days) Second Week (Two injections) Third week to seventh week - once a week. Total 10 injections. Thank you .

Have a Great Day. Yes, I have stopped all those drugs.I am continuing on Montair LC now. When I use Montair, I get very good sleep compared to when I am not taking it. So I guess one of my major reasons for my wheezing is allergy. The place, where I currently live is surrounded by villages, most villages have this parthenium in large. And even around my house there is a huge amount of parthenia. Thanks about your idea of vaccination. Thats a good idea. my doubt is if we use that, will I get moderate or severe wheezing? Should I do that in a hospital getting admitted? Please let me know. Recently when I was admitted, I was given the following drugs. I am attaching those drug bills which has all the names for your review. The doctor told me that he is injecting a vaccine, and it may prevent wheezing for next one to one and half year.
